- This is a SOURCES SOUGHT / REQUEST FOR INFORMATION (RFI) announcement and is in accordance with FAR Subpart 15.201(e). �It is neither a solicitation announcement nor a request for proposal (RFP) or request for quote (RFQ) and it does not obligate the Government to award a contract.� Request for a solicitation will not receive a response.� The purpose of this sources sought announcement is for market research to make the appropriate acquisition decisions, to include set aside decisions and to gain knowledge of potential qualified sources and their Small Business Administration (SBA) size classification in relation to North American Industry Classification System (NAICS) 621340 � Offices of Physical, Occupational and Speech Therapists, and Audiologists The United States Department of Agriculture (USDA) Food Safety and Inspection Service (FSIS) Office of Operations (OFO) is seeking interested sources who can provide Nationwide Mobile Audiometric Testing services for all Consumer Safety Inspectors (CSI) at various processing and slaughter plants (please use this link to see all district locations: https://www.fsis.usda.gov/contactus/fsis-offices/office-field-operations-ofo.��� The contractor shall provide management, supervision, labor, materials, equipment, supplies, certifications, and any other additional resources to provide onsite mobile audiometric testing at the request of OFO.��� The contractor must be familiar with, have an understanding of and must comply with the various requirements in Occupational Safety and Health Administration (OSHA) standards 29 CFR 1910.95.� All technicians must be trained and certified by the Council for Accreditation in Occupational Hearing Conservation (CAOHC) and be under the supervision of a Licensed Certified Audiologist.� All testing and calibration procedures as well as record keeping need to be in compliance with applicable OSHA and other regulatory requirements. �The tests will support compliance with the applicable provisions of 29 CFR 1910.95, Occupational Noise Exposure with regard to personnel, testing procedures and facilities/equipment.� �Contractor must also adhere to CDC, local, state, and Federal standards for protecting against transmission of COVID-19.� If you are interested and are capable of meeting this requirement, please provide the requested information indicated below. 1.� Company Name: 2.� Address: 3.� Point of Contact (name/email/phone): 4.� DUNS Number: 5.� GSA Contract No. and expiration date (if applicable): 6.� Contract number(s) and point of contact of any current / past contracts of providing these services with any Federal Agency. 7.� Small Business Designation (i.e. SDVOSB, VOSB, WOSB, etc): 8.� Please submit a brief 1 � 5 page capabilities statement to show how your company is capable of meeting this requirement.� Responses to this notice shall be submitted via email to Reggie Brown, Contract Specialist at reginald.brown@usda.gov and shall include �123A9422R0004 � Nationwide Mobile Audiometric Testing - (Contractor Name)� as the subject line of your email response.
