Description
SOURCES SOUGHT NOTICE CARDIOVASCULAR/THORACIC SURGEON SERVICES The Great Lakes Acquisition Center (GLAC), on behalf of the Captain James A. Lovell Federal Health Care Center (FHCC), 3001 N. Green Bay Road, North Chicago, Illinois, 60064 (hereinafter called FHCC), is seeking contractors to provide Cardiovascular/thoracic Surgeon Services for the FHCC. The projected period of performance will be August 1, 2022 through July 31, 2027. Contractor shall provide Cardiovascular/thoracic surgeon on-call coverage 24 hours per day/7 days a week, and one, three-hour clinic per week, other elective/emergent intermediate surgical procedures, and call-back, for the FHCC. All clinics will be performed at the FHCC. Elective intermediate surgical procedures will be performed at the FHCC. Emergency services will be performed off-site and are not included as part of this contract. Contractor shall be available to the FHCC to either perform or assist in Cardiovascular and Cardiothoracic surgical procedures whether routine or emergency cases. Contractor will also assist on other cases when appropriate. When the on-call provider is paged, they shall contact the FHCC within 15 minutes of being paged to be available for consultation for patient assessment. If a patient s condition requires the physical presence of the on-call provider, the provider must be able to report to the FHCC within 30 minutes of the end of the phone consultation. All consults and admissions will be seen within 24 hours or sooner as required by the patient s clinical condition. In addition, contractor shall come to the FHCC when necessary to make rounds and document patient treatment in the electronic medical record, as well as, follow-up on results from diagnostic studies (labs, imaging, etc.) and medications that have been ordered or require follow-up. Contractor shall comply with FHCC medical record requirements and adhere to FHCC surgery staff policies. Progress notes shall be completed within the same shift or next visit. Contractor shall participate in performance improvement activities as required by the Cardiovascular and Cardiothoracic Section of the FHCC Surgical Service. Personnel assigned by the contractor to perform the services covered by this contract shall have a full and unrestricted license in a State, Territory, or Commonwealth of the United States or the District of Columbia. Physicians must be either a Board Eligible or Board-Certified Cardiovascular Surgeon or Board Eligible or Board Certified Cardiothoracic Surgeon trained in Vascular Surgery. Physicians must also possess and maintain a current BLS certification during the life of this contract. The qualifications of such personnel shall also be subject to review by the FHCC Chief of Staff and approved by the FHCC Facility Director. The Contracting Officer will be notified prior to any changes in personnel. FHCC reserves the right to approve the assignment of individual personnel furnished by the contractor to perform the functions specified in the contract. Personnel assigned by the contractor to perform the Cardiovascular/Thoracic Surgeon covered by this contract shall also be proficient in written and spoken English (38 USC 7402). Personnel assigned by the contractor to perform the Cardiovascular/Thoracic Surgeon Services covered by this contract will be subject to credentialing and an Access National Agency Check with Written Inquiries (ANACI) prior to providing services. The NAICS Code for this procurement is 621111; size standard $12.0 million. The Service Contract Labor Standards will also apply to this procurement. This Sources Sought Notice is for market research purposes only and shall not be considered as a request for quote or an obligation on the part of the Government to acquire any services. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of the contractor submission of this Sources Sought Notice or the Government s use of such information. The Government reserves the right to reject in whole, or in part, any contractor s input resulting from this Sources Sought Notice. No contractual arrangement will be made as a result of this notice. Information submitted in response to this Sources Sought Notice will not be returned. All interested parties that are capable of providing the required services shall submit a response via email to sandra.mcanany@va.gov and georgiann.schneider@va.gov no later than April 4, 2022 at 10:00 a.m. central time. The responses shall address the information below. Contractor names, DUNS number, address, point of contact, title, phone number, and email address Brief description of the company background, capability to meet this requirement (may include nurse resumes, facility/company accreditations, type of equipment used, etc.), and past or current performance under contract (civilian or government). Contractor s socioeconomic status, including Vendor Information Pages (VIP) verification, if applicable. Copy of current certificate of liability insurance. Contractor s interest in responding to a solicitation if issued.
