- Page 2 of 2 REQUEST FOR QUOTES: 36C26222Q0613 PPROJECT TITLE: Veterans Health Administration Tucson Arizona Healthcare System Refrigerators This is a combined synopsis/solicitation set aside for Service-Disabled Veteran Owned Small Businesses (SDVOSB) for commercial supplies/services prepared in accordance with the format in subpart 12.6, as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide the Tucson, Arizona Healthcare System with Brand name or equal: 36C26222Q0613 Roll-In Refrigerators and Undercounter Refrigerator/Freezers Nutrition and Food Service/Dietitian Interested SDVOSB vendors/suppliers/dealers shall provide all materials, transportation, delivery, and supervision necessary to satisfy the needs of the ordering activity. The successful SDVOSB must provide the specified product that meets or exceeds the RFQ specifications. Shipment and delivery must be arranged with the Nutrition and Food Service/Dietitian. The Contractor shall furnish all materials and service to the VA Tucson Arizona Healthcare System, Tucson, Arizona 85723. The solicitation will be in accordance with FAR Part 12, 13, and 19, Acquisition of Commercial Items, Simplified Acquisition Procedures, Small Business Programs, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. The North American Industrial Classification System (NAICS) code for this requirement is 333415 size standard 1250 employees. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract. The Contract begins on the date of award. Due to time constraints, responses to the solicitation shall be due on March 30, 2022, at 1:00 PM PST. Offerors are advised that it is your responsibility to review and monitor the website frequently to ensure you have the most up to date information, including amendments. All offerors must be registered in www.sam.gov to do business with the Government. Ensure your registration is correct, current and has not expired. Note: Re-manufactures or gray market and/or counterfeit items/ supplies will NOT be accepted. The items/supplies must be O.E.M. (Original Equipment Manufacturer) only. Only authorized Steelcase resellers, distributors, dealers or manufacturers will be considered as potential sources for market research purposes. If you are a small business and authorized distributor/reseller to the VA for the items identified above (or equivalent product/solution), please submit letter/documentation stating authorization/partnering from the manufacturer for bid to be considered for selection. FAR 52.212-1, Instructions to Offerors Commercial Items ADDENDUM: Applies to this acquisition and the following addenda to the provision. The Offeror s quotation shall be submitted electronically on company letterhead stationery to the Points of Contact identified in Contract Opportunities combined synopsis/solicitation announcement by the date and time indicated. As part of your quote submission, you are required to include price information. DUNS, Cage Code, tax ID, terms and conditions and Buy American Act Certificate required in clause 252.225-7000, as applicable. Late submissions Offerors are responsible for submitting at the time specified. An offer received after the time specified is late and will not be considered. In accordance with FAR 52.204-7 an offeror is required to be registered SAM at https://www.sam.gov at time of quote submittal in order to be considered for award. Please provide quote expiration date and best delivery schedule. 852.219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (a) Definition. For the Department of Veterans Affairs, Service-disabled Veteran-owned small business concern or SDVSOB: (1) Means a small business concern (i) Not less than 51 percent of which is owned by one or more service-disabled Veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled Veterans or eligible surviving spouses (see VAAR 802.201, Surviving Spouse definition); (ii) The management and daily business operations of which are controlled by one or more service-disabled Veterans (or eligible surviving spouses) or, in the case of a service-disabled Veteran with permanent and severe disability, the spouse or permanent caregiver of such Veteran; (iii) The business meets Federal small business size standards for the applicable North American Industry Classification System (NAICS) code identified in the solicitation document; (iv) The business has been verified for ownership and control pursuant to 38 CFR 74 and is so listed in the Vendor Information Pages database, (VIP Home · VetBiz Portal (va.gov); and (v) The business will comply with VAAR subpart 819.70 and Small Business Administration (SBA) regulations regarding small business size and government contracting programs at 13 CFR part 121 and 125, including the nonmanufacturer rule and limitations on subcontracting requirements in 13 CFR 121.406 and 125.6, provided that any reference therein to a service-disabled veteran-owned small business concern (SDVO SBC), is to be construed to apply to a VA verified and VIP-listed SDVOSB. The nonmanufacturer rule and the limitations on subcontracting apply to all SDVOSB and VOSB set-asides and sole source contracts. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: The award will be made to the response most advantageous to the Government. Brand name (or equal) and price. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
