- This is a SPECIAL NOTICE only. Responses to this notice will be used for information and planning purposes only. No quotes are being requested or accepted at this time with this notice. The Department of Veterans Affairs, Lebanon VA Medical Center located in Lebanon, PA has a requirement for County Mobile Data Terminal (MDT) Service. Potential Sources shall respond with their ability to provide these services as outlined in the draft Statement of Work (SOW) The North American Industry Classification System (NAICS) code being considered for this procurement is 561621: Security Systems Services (except Locksmiths) (PSC): DA01 IT and Telecom Business Application/Application Development Support Services. Responses to this notice should include the following: Company name Unique Entity Identifier (UEI) SAM number Company s address, Point of Contact information title, phone number and email address Company s detailed Capability Statement Shall detail companies abilities to meet this requirement and any pertinent information which demonstrates that ability Provide GSA Federal Supply Schedule (FSS) contract number, NASA SEWP contract number, or other applicable contract vehicle contract number, if within scope of this effort (if applicable). Provide SAM Registration Verification Provide VIP verification provided, if applicable Provide Company s type of business (small or large) and socioeconomic status whether Service Disable Veteran Owned, Veteran Owned, Hubzone, 8(a), Women Owned, Small Disadvantaged, etc. If applicable, Service-Disabled Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB) must be verified in www.VetBiz.gov. Response should provide enough information to determine if the vendor can comply with the limitations on subcontracting as defined in VAAR 852.219-74 and all applicable CFR references where applicable. Non-developer/manufacturers shall provide documentation that they are authorized resellers/distributors of this requirement. All responses shall be submitted in writing via email no later than 11:00 (EST) April 14, 2022 to Contract Specialist, Antonio Armstrong at antonio.armstrong2@va.gov. Responding as an interested party does not constitute your company as a viable Source. After review of the responses to this Special Notice, a solicitation announcement may be published on the Beta.SAM (Contract Opportunities), GSA and/or NASA SEWP websites. Responses to this SPECIAL NOTICE announcement are not considered to be adequate responses to any future solicitation announcement. All interested parties will have to also respond to any future forthcoming solicitation announcement, in addition to responding to this Special notice. All interested parties must be registered with System for Award Management (SAM) at https://www.sam.gov to be eligible for award of Government contracts. Draft STATEMENT OF WORK Mobile Data Terminal (MDT) Service Contract GENERAL INFORMATION: TITLE OF PROJECT: County Mobile Data Terminal (MDT) Service Contract SCOPE OF WORK: Department of Veterans Affairs (VA), Lebanon VA Medical Center, located at 1700 S. Lincoln Ave, Lebanon, PA 17042, has a Federal VA Police Service on the property. This Service Line interacts with the Lebanon County Department of Emergency Services (DES) and relies on the DES for county dispatch services and the use of county Mobile Data Terminals (MDTs). The Lebanon VA Police currently has four (4) MDTs, one in each of the marked VA police vehicles. The MDT is the computer link between the VA Police and the County DES. The MDTs allow the VA Police and the County Dispatch to communicate, back and forth for criminal histories, driver licenses checks, vehicle registration, wants & warrants, drivers histories, etc. during traffic stops, criminal investigations, and accident responses. Without this computer link the VA Police Officers responding to or conducting their Law Enforcement mission would not be able to complete this mission safely. BACKGROUND: VA Police currently utilize Lebanon County MDTs and dispatching services through the MDTs to perform site specific (Lebanon County) police enforcement operations. The services provided by the Lebanon DEM is required to operate the MDTs which are installed in all our marked patrol vehicles. PERFORMANCE PERIOD: BASE YEAR May 1, 2022 through December 31, 2022 OPTION YEAR 1 January 1, 2023 through December 31, 2023 OPTION YEAR 2 January 1, 2024 through December 31, 2024 OPTION YEAR 3 January 1, 2025 through December 31, 2025 OPTION YEAR 4 January 1, 2026 through December 31, 2026 B. GENERAL REQUIREMENTS: Lebanon County DES will provide MDT services in the way of providing MDT terminals and allowing access on the Pennsylvania Justice Network (JNET)/CLEAN system. D. COUNTY RESPONSIBILITIES: Lebanon County shall provide wireless operational services to all issued MDTs. E. GOVERNMENT RESPONSIBILITIES: Payment for operational services on a yearly basis. F. OTHER CONSIDERATIONS: CONTRACTING OFFICER REPRESENTATIVE (COR): COR shall be the technical point of contact for all work-related requirements. COR does not have authorization to change, alter, or remove any requirements stated in the performance work statement. Contracting Officer is the only government authorized person to change the performance work statement or any resultant contract. PLACE OF PERFORMANCE: Wireless signal transmitted by the Lebanon County DES to VA Police MDTs. WORK HOURS: Twenty-four (24) hours a day, three hundred sixty-five (365) days a year. DELIVERY: Operational wireless communications with Lebanon County and State Law Enforcement Agencies. HOURS OF OPERATION: Services covered by this contract shall be furnished by the County DES as defined herein. VHA Supplemental Contract Requirements for Combatting COVID-19 1. Contractor employees who work in or travel to VHA locations must comply with the following: a. Documentation requirements: 1) If fully vaccinated, contractors shall show proof of vaccination. i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. 2) If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. 3) Documentation cited in this section shall be digitally or physically maintained on each Contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. 4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. 2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the https://www.va.gov/covid19screen/. Check regularly for updates. a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the https://www.va.gov/covid19screen/, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID- 9 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. 3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. 4. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
