Description
SOW Contract Title. 2 fire doors with all al the hardware for installation. 3. Background. The intent of this contract is for the Cincinnati VA Medical Center to purchase TWO (2) 3 - 7 15/16 X 6 - 11 7/8 ULC RNB 4 X 25 LO PRO KIT FIRE LITE NT GLASS HALF MORTISE EDGE GUARDS CLEAR FINISH RHR WOOD DOORS and (2) CORBIN RUSSWIN ED5470B W048 H0700 M55 630 RR SURFACE VERTICAL ROD DEVICES, (6) HAGER BB1168 4.5 X 4.5 US26D FULL MORTISE HINGES, (1) ALARM LOCK ETDL2700 T2 LC US26D TRILOGY EXIT TRIM, and (2) ROCKWOOD K4125 16"" X 41"" CLEAR PLASTIC PROTECTION PLATE for the VA medical center here in Cincinnati, Ohio. 4. Scope. This contract is for the VAMC, Cincinnati, Ohio Engineering department for the Carpentry shop to install, it is expected that the equipment will be fully functional upon delivery to the VAMC. 5. Specific Tasks. Purchase TWO (2) 3 - 7 15/16 X 6 - 11 7/8 ULC RNB 4 X 25 LO PRO KIT FIRE LITE NT GLASS HALF MORTISE EDGE GUARDS CLEAR FINISH RHR WOOD DOORS and two (2) CORBIN RUSSWIN ED5470B W048 H0700 M55 630 RR SURFACE VERTICAL ROD DEVICES, six (6) HAGER BB1168 4.5 X 4.5 US26D FULL MORTISE HINGES, one (1) ALARM LOCK ETDL2700 T2 LC US26D TRILOGY EXIT TRIM, and two (2) ROCKWOOD K4125 16"" X 41"" CLEAR PLASTIC PROTECTION PLATES. 6. Performance Monitoring Inspection will be performed upon delivery by engineering Carpentry shop, once completed, Staff operating products will monitor equipment for failures. Premature failures will be reported to the distributor and manufacturer, for quality control corrections as well as replacement. PM s (fire door inspections) will be done by staff. 7. Security Requirements This equipment does not store patient data or other health care information. However, will have to be supplied with operating keys. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). None. 9. Other Pertinent Information or Special Considerations. None. a. Identification of Possible Follow-on Work. None. b. Identification of Potential Conflicts of Interest (COI). None. c. Identification of Non-Disclosure Requirements. None. d. Packaging, Packing and Shipping Instructions. Entire purchase will be delivered/installed to the Cincinnati VA Medical Center/Engineering department. Engineering specifically the Carpentry shop will need to perform a safety check, and an acceptance inspection upon receipt, and enter them into the system for Fire door inspections. e.
