- (I)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation for this action. Quotes are being requested for preventative maintenance at the Richmond VAMC. A written solicitation will not be issued. (II)This solicitation is issued as a Request for Quote (RFQ). Submit written quotes on RFQ 36C24622Q0465_1 (III)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-05. (IV) Any amendments issued to this combined/synopsis will ONLY be available on the SAM website (SAM.GOV). (V) The Network Contracting Office 6-RPO East intends to award a firm-fixed-price contract for the VAMC RICHMOND VA. (VI) The North American Industrial Classification System (NAICS) code for this procurement is 811219 with a small business size standard of 500 employees. This procurement is a set-aside for SDVOSB, and all eligible offerors may submit quotes. All offerors must be registered in the System for Award Management (SAM) at http://www.sam.gov to be considered for an award of a federal contract. The record must be complete and not missing elements such as representations and certifications. All SDVOSBs must be listed and registered in Vetbiz.gov to be considered for award. (VII) Questions concerning this solicitation shall be addressed to Audrey Wooten Contracting Officer and emailed to Audrey.Wooten@va.gov. All questions or inquiries must be submitted no later than 9 am Eastern Standard time on 05/20/2022. No phone call will be accepted. (VIII) The Government will award a contract resulting from this RFQ to the responsible offeror whose quote MEETS SOW and Items LISTED IN PRICE SCHEDULE. Quotes are due by 10 am Eastern Standard time on 05/23/2021 Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Quotes shall be submitted via email to Audrey.wooten@va.gov. (IX) This combined solicitation/synopsis is for the purchase of the following commercial items: ITEM INFORMATION The Richmond VA Medical Center, located at 1201 Broad Rock Blvd, Richmond, VA 23249 is seeking a potential qualified contractor who can provide the following service stated in the SOW and PRICE SCHEDULE (no gray market items accepted): Statement of Work Contract Title Steris Reliance 6000 Series Endoscopy Cabinets Annual Preventative Maintenance 3. Background. Units are used to safely stow sterile scopes used for patient care. 16 Steris Reliance 6000 series Endoscopy Storage Cabinets require annual preventative maintenance. 4. Scope. 16 endoscopy cabinets require annual preventative maintenance which shall include replacement of filters and software accessed to reset filter timers. The servicer will come to Richmond VA Medical Center to complete annual maintenance. Servicers will need to comply with operating room (OR) clean techniques to enter sterile areas where units are located. 5. Specific Tasks. Service technicians shall replace all filters and reset software for 16 Steris Reliance 6000 Series endoscopy cabinets. Units are located in the sterile areas of the hospital s ORs, Sterile Processing Department, and 2D Urology Department. Original equipment manufacturer OEM parts shall be used and brought on-site with the service technician at the time of service. The service date shall be scheduled with the Biomedical (BIOMED) Engineering Department. No additional maintenance is required by the service technician. Documentation of annual preventative maintenance services shall be given to a BIOMED engineer upon completion. Any findings of damaged units shall be documented and reported to a BIOMED engineer. The BIOMED Engineering Department will complete any additional repair services outside of the scope of annual preventative maintenance. (End of clause) VAAR 852.219-77 VA NOTICE OF LIMITATIONS ON SUBCONTRACTING CERTIFICATE OF COMPLIANCE FOR SERVICES AND CONSTRUCTION (SEP 2021) (DEVIATION) (a) Pursuant to 38 U.S.C. 8127(k)(2), the offeror certifies that (1) If awarded a contract (see FAR 2.101 definition), it will comply with the limitations on subcontracting requirement as provided in the solicitation and the resultant contract, as follows: (i) [x] Services. In the case of a contract for services (except construction), the contractor will not pay more than 50% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs as set forth in 852.219-10 or VOSBs as set forth in 852.219-11. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. The test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA-approved over-the-counter test. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening, process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
