- Source Sought This is a sources sought notice only and it is not a solicitation announcement. The purpose of this sources sought notice is to gain knowledge of qualified, potential contractors and their socio-economic size classification in accordance with NAICS code 339113 (Surgical Appliance and Supplies Manufacturing) responses will be used by the Government to make appropriate acquisition decisions. After a review of the responses to this sources sought notice, a solicitation announcement may be posted. A response to this sources sought notice is not an adequate reply to a potentially forthcoming solicitation. The Department of Veterans Affairs, (NCO 08), 13000 Bruce B. Downs, Tampa, Florida 33612 is seeking authorized Service Disabled Veteran Owned Small Business (SDVOSB s) , Veteran Owned Small Business (VOSB s) that can provide exam tables brand name or equal to Midmark product numbers: 626-004, 002-2011-863, 9A600001, 9A610001, 647-003, 002-10129-863. The vendor shall deliver to New Port Richey Community Based Outpatient Clinic in New Port Richey Florida. SALIENT CHARACTERISTIC S Quantity 90 each. Brand name or equal to: Midmark 626-004 (PN: 626-004) Barrier-Free Exam Table to include the following: Patient Weight Capacity 650lbs, and the table must be able to lift the capacity weight identified. Table must be 76 long (+- 1 ) with footrest extended. Powered height and backrest adjustable are required for safety and timeliness of patient exams. Must have safety feature to detect object if lowering to prevent injury. Hospital-grade receptacles Electrical requirement: 115VAC, 50/60 Hz, 12A Must be able to lower to a height of 15.5 (+- 0.5 ) for optimal patient safety. This allows patients to safely sit on the table without the use of step stools which increase risk of fall/injury. This specific height also allows most patients to have their feet flat on the floor which is the medically efficient way to take an accurate blood pressure. Must be able to raise to a height of 37 (+- 1 ) for the providers ergonomics and safety when performing exams and procedures. The table must be able to lay flat at all heights for the safety of patient transfer if necessary. The table must be able to be put into a chair position at any height for efficient exam purposes. Must have a built-in digital scale with a 650lb capacity. This allows providers to capture accurate weight from patient population who struggle with standing. This also eliminates the need for unnecessary patient transfer, which reduces the risk of fall/injury for patient as well as providers. Table base rollers are required and must be retractable. This option allows for the table to be moved easily for cleaning and infection prevention. Easy movement of the tables ensures the safety of the staff. Retractable wheelbase is required to ensure that the table sits securely on the floor and accidently movement eliminated. Must have wireless hand and foot controllers to ensure that wires are not hindering walking area, which ensures patient and provider safety. Must have safety feature to allow table to be lowered if power outage. Table must have 7-degree pelvic tilt (+- 1 degree) Storage drawer must be available and heated for storage of providers exam instruments. This is necessary for patient comfort. Paper roll storage required Required to have multi-positional stirrups which can be slid inside the table for storage. Quantity 90 each. Brand name or equal to: Midmark Upholstery Top Midmark 626 (PN 002-2011-863) 28 Heated Premium Removable seamless microbial upholstery for infection prevention and cleaning ease. Heated upholstery for patient comfort Quantity 90 each. Brand name or equal to: Midmark Patient Support Rails Midmark (PN: 9A600001) Must have patient support rails option to ensure the proper and efficient positioning of the patients arm for capturing blood pressure measurements. Quantity 90 each. Brand name or equal to: Midmark Stainless Steel Treatment Pan (PN: 9A61001) Must have stainless steel treatment pan for exams and procedures. This allows for less space to be taken up in the room for safety. Quantity 90 each. Brand name or equal to: Midmark 647-003 (PN: 647-003) Podiatry Procedure Chair Patient Weight Capacity 450lbs, and the table must be able to lift the capacity weight identified. Hospital-grade receptacles. Powered height and backrest adjustable are required for safety and timeliness of patient exams. Must have safety feature to detect object if lowering to prevent injury. Exam Chair must be 84 long (+- 1 ) with footrest extended. Chair must have base height with minimum 19 / max 40in (+- 1 ) Must have an extra wide patient surface for to allow for ease of patient transfer and safety. Chair arms must be removable and able to be adjusted laterally for patient transfer/maneuver. The chair must be able to lay flat at all heights for the safety of patient transfer if necessary. Foot section must be able to have range from 15.5 to 57 high for efficient provider exam and care. Debris tray must be provided for exam and procedure cleanliness and efficiency. Quantity 90 each. Brand name or equal to: Midmark Upholstery Top 647 Premium 28 (PN: 002-10129-863) Removable seamless microbial upholstery for infection prevention and cleaning ease.
