Description
Radiometer ABL90 Flex Plus Supply and Service Statement of Work Equipment and Supply Requirement:  Equipment Requirement: No equipment is required: SFVAHCS owns two ABL90 Flex Plus Instruments and only requires supply and service for these instruments. Currently owned equipment: Serial 092R0246N097 Serial 092R0246N098 Equipment must be maintained, serviced, and replaced upon failure. Deliver to: San Francisco VA Healthcare System, Laboratory Medicine Service, Microbiology Section (Room 2B113, building 200), 4150 Clement Street, San Francisco, CA 94121 Period of Performance: Base Year: October 1, 2022 through September 30, 2023 Option Year 1: October 1, 2023 through September 30, 2024 Option Year 2: October 1, 2024 through September 30, 2025 Option Year 3: October 1, 2025 through September 30, 2026 Option Year 4: October 1, 2026 through September 30, 2027 Supply requirements: The following reagents and quantities are required for each year of the contract: 944-157 ABL90 FLEX solution pack 24 946-010 SC90 100/30 BG/LYT/MET/OXI +QC 24 944-021 tHb Calibration (4 amp.) 2 984-070 Thermal Paper, 8 rolls per unit 2 VK-R5 Linearity Verification Kit 2 834-662 Inlet connector gasket, ABL90 Flex 2 903-585 Inlet gasket with holder, ABL90 Flex PLUS 2 944-017 Qaulicheck5+ level1 Ref 1 Delivery must be arranged with the testing laboratory. Ship schedules based on usage are preferred, however, ordering as needed should be available to ensure uninterrupted testing. System Service/Support/software: During the Equipment installation, service team can assist in customized analyzer device installation, data management, and connectivity. Upon Equipment installation, key operator and operator on-site training will be provided. One (1) annual service maintenance, which includes cleaning, lubrication, alignment and replacement of predetermined parts using a Radiometer yearly maintenance kit. Up to one (1) inlet probe, three (3) inlet gaskets, and one (1) Verification Kit per year for use during a scheduled service visit. Additional six (6) month maintenance visits with tubing replacement. Complete documentation will be provided on all maintenance work. No-charge software updates and upgrades will be provided during the term of the Agreement. Phone support provided as initial step until the determination is made that an onsite visit is required. Phone/Hotline provided 24/7. Telephone response time is within one (1) business hour from the initial customer call. If determined the problem cannot be resolved remotely, onsite service visits will be provided Monday through Friday 8 a.m. to 8 p.m. local time. All labor, travel and parts are included. If applicable, the use of loan equipment for a charge (ABL80 Series and ABL90 FLEX only). Priority onsite support is provided within twenty-four (24) hours (Monday through Friday 8 a.m. to 8 p.m. local time) from the time Radiometer determines an onsite visit is required. Customer can access MyRadiometer.com and AcuteCareTesting.org. Customer will have access to product operator manuals, clinical training, and mobile apps for Blood Gas and Pre-analytical errors. All parts warrantied for 90 days; Labor 30 days. Annual key operator training (up to five trainees) and access to product e-Learnings modules. Live Connect activated at no charge. Live Connect Performance Insights and Remote Reporting (Live Connect activation required). Annual instrument performance review as requested. Cyber security package (upon availability). Invoicing: The contractor shall send a proper invoice to Tungsten Network. Facsimile, e-mail, and scanned documents are not acceptable forms of submission for payment requests. Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods below: VA s Electronic Invoice Presentment and Payment System The FSC uses a third-party contractor, Tungsten Network, to transition vendors from paper to electronic invoice submission.
