Description
Sources Sought Note: This is not a solicitation but rather a Request for Information (RFI/Sources Sought) for market research purposes only. Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office 5, is issuing this Sources Sought in order to identify capable firms and obtain information for planning purposes only. Responses must be submitted by 2:00 PM EST, June 23, 2022. Submit responses to this Sources Sought via email to theresa.thomas7@va.gov All SDVOSB and VOSB firms that respond shall include proof of CVE certification via www.vip.vetbiz.gov. All small business firms that respond shall include proof of small business status via their Representations and Certifications in accordance with (FAR 4.1102 Policy). While SDVOSB/VOSB firms are preferred, all capable firms are welcome to respond to this Sources Sought. All respondents to this Sources Sought shall include as part of their response answers to the below questions and provide any additional pertinent information that supports the answers. If there are any ambiguities in this Sources Sought, please identify them in your response. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., socioeconomic set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses. Suggested NAICS: 332312 (Fabricated Structural Metal Manufacturing) Suggested PSC: 5450 (Miscellaneous Prefabricated Structures) Open to suggestions from the market as to a more proper NAICS and/or PSC, as well as any potential GSA/FSS Schedule SIN categories. If you are interested, please e-mail the following information: Company Name. Person of Contact (including telephone number and email address). Socio-economic status (SDVOSB, VOSB, Woman-Owned, Other etc.). Do you currently hold a government contract? Contract Number? Type of contract? With whom (GSA, SEWP, etc )? If GSA what Schedule and SIN? Can your company provide the service for the listed Department of Veterans Affairs locations? Does your firm utilize the suggested NAICS and PSC per this Sources Sought Notice? If not, please suggest a more appropriate NAICS and/or PSC and explain why it s more appropriate? How do you invoice for this service, including rates? What Certifications, State and/or Federal, if any must you have to perform this service? Please confirm that your firm s contract performance would be in compliance with the Limitations on Subcontracting as required by 13 CFR 125.6 and VAAR 852.219-74 if applicable (Not applicable to unrestricted set aside). Please indicate in which clause your performance would be in compliance with: VAAR 852.219-10, VAAR 852.219-11, or FAR 52.219-14. (Attachment B) Please list all of your concerns, questions and ambiguities below. Questions will not be addressed directly but will be used by the Government in the event that this Sources Sought would move forward to solicitation. Please provide estimated rates. STATEMENT OF WORK Eckel Audiology Booths Brand name or Equal to A. GENERAL: Scope of Work: The Martinsburg VA Medical Center has a requirement for replacement audiology booths to include all parts and labor for the furnishing of the booths. The requirement includes one sound room, one L-shaped sound room, installation of the booths, and removal of the existing booths. WORK HOURS: Hours of Operation: Contractor shall complete work outside of normal business hours of 8 a.m. to 4:30 p.m. to allow care to continue in the intact audiology booth adjacent to the booths requiring replacement. National Holidays: The ten holidays observed by the Federal Government are: New Year s Day, Martin Luther King s Birthday, President s Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving Day and Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. SPECIFICATIONS: The requirement is for one (1) Eckel Custom Sound Room in an L shape double walled suites configuration. The requirement is for one (1) Eckel Custom Sound Room. The booth shall all be double walled. Contractor shall have drawings for the facility and be familiar with the existing conditions to ensure fit/function. Deliverables: Item Qty Eckel Custom Sound Room 22P012-D1 L shape double walled suites 1 Eckel Custom Sound Room 22P012-D2 CL-124 MOD 1 Installation/De-Install Sound Rooms: -Tear down existing booths and dispose in facility dumpster. -Install new booths. 1 Tasks: Consultation and planning services for the audiology booth project. Deliver all equipment to Martinsburg VA Medical Center, Building 500 Warehouse, 510 Butler Avenue, Martinsburg, WV 25405. Perform deinstallation of the existing booths and dispose of the booths. Install at Martinsburg VAMC, 510 Butler Avenue, Martinsburg, WV 25405 in the Audiology Suite. Maintain all dust/debris inside barriers to prevent infection. New caulking and replacement acoustic filler shall be used to seal all panel joints. Complete work outside of normal business hours. Personal Protection Equipment (PPE): Contractors shall be responsible for their own PPE when performing work on-site. Masks shall be worn in all healthcare areas. Screening: All persons entering the facility shall be screened by contracted screeners based on current CDC (Center for Disease Control) guidelines. Contract personnel are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. QUALITY ASSURANCE The Government will inspect and test that the items are installed properly and meet the function required. E.
