Description
THIS IS A SOURCES SOUGHT NOTICE ONLY Sources Sought Notice: This Sources Sought Notice is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this is marked as proprietary will be handled accordingly. The purpose of this notice is to obtain information on CVE VetBiz VERIFIED Service-Disabled Veteran Owned Small Businesses (SDVOSB) and Veteran Owned Small Businesses (VOSB), registered on VIP Home  · VetBiz Portal (va.gov), and other Small Business (SB) interests, capabilities, and qualifications to determine if a set-aside is appropriate. Responses to this sources-sought announcement will be used by the Government to make an appropriate decision. Proposals are NOT being requested or accepted at this time. DESCRIPTION: Project 556-23-100 Campus-Wide Land and Utility Survey The Captain James A. Lovell Federal Health Care Center (FHCC), located at 3001 Green Bay Road North Chicago, IL 60064, has a requirement to obtain campus-wide professional land surveying services to locate and record boundaries, structure data, and utilities above and below ground, across approximately 108 acres, and approximately 64 individual structures totaling 1.8 million square feet, some dating back to 1925, but with significant clinical facilities of modern construction. The survey shall include property lines, roads, lots, parcels and easements, surface topography, and the square footage and elevation for each building. The survey shall include all utility systems, identifying all parts of each whether underground or pole-mounted. Utility systems include campus domestic water, fire hydrants, sanitary sewer, storm sewer, steam transmission and condensate return piping, inter-building fiberoptic and other telecommunications cable including that serving security systems, electrical power distribution, and campus lighting. Services shall include, but not be limited to, the furnishing of all materials, labor, equipment, tools, supervision, and miscellaneous items necessary to complete this requirement. The work shall be completed during normal business hours. Any work needing to occur outside normal business hours shall be included in the Surveyor s proposal, and will require advance approval by the Contracting Officer s Representative (COR). The Contractor shall not be admitted to areas of the property other than those designated for this project. The Contractor shall remain solely responsible for providing, installing, and maintaining all safety barricades and signage needed to keep its work area safe and separate from pedestrian and vehicular traffic. Well in advance of need, the Contractor is required to coordinate and collaborate through the VA COR with the VA Police Department at this facility to develop and gain approval of a multi-phased traffic safety plan. RESPONSE INFORMATION: The following information is requested in response to this Notice from interested firms. Interested SDVOSB, VOSB, and/or SB Firms must indicate their statement of intent to submit a proposal (SF 330) in response to a potential solicitation, via email only, to Contracting Officer Kristi Kluck, Kristi.Kluck3@va.gov by August 23, 2022, at 4:30 p.m. CST. Interested Firms shall provide Company name, address, point of contact, phone number, DUNS number, SAM UID number and business status. Identify self-performance, any Teaming Agreement or Arrangement, and/or intention of subcontracting. Interested Firms shall provide clear evidence of your Firm s experience in performing surveying and mapping services of the surface of the earth. These services include surveying and mapping of areas above or below the surface of the earth, such as the creation of view easements or segregating rights in parcels of land by creating underground utility easements. When providing evidence of your Firm s experience, provide description of projects and contracts similar in scope, size, and complexity successfully completed by your Firm, include total dollar amounts of those completed projects/contracts, and telephone numbers for points of contacts for those projects (no photographs or drawings are needed at this time). Responses should be for recent experience (within 5 years). IAW 52.219-14 Limitations on Subcontracting Clause, must provide evidence of your Firm s ability to perform 50% of the requirement. All information must be submitted in sufficient detail for a decision to be made on the availability of interested SDVOSB, VOSB or SB concerns. Prime contractor should have registered architect and engineers as members of firm or team. Failure to submit all information requested will result in a Firm not being considered as an interested SDVOSB, VOSB or SB concern. If adequate interest is not received this action will not be a set-aside. Total submittal shall be no longer than 15 pages in one .pdf file or word document. Responses are required to be sent via email to Kristi.Kluck3@va.gov no later than Tuesday, August 23, 2022, 4:30 p.m. CST. Firms must be registered in the System for Award Management (SAM). Interested Firms can register at https://www.sam.gov/content/home. SDVOSB and VOSB firms must be CVE registered/verified by the Center for Veteran Enterprises (CVE). NOTE THAT THIS IS NOT A REQUEST FOR PROPOSAL. THE GOVERNMENT IS SEEKING INFORMATION FOR MARKET RESEARCH PURPOSES ONLY. THE GOVERNMENT MAY OR MAY NOT ISSUE SOLICITATION DOCUMENTS (REQUEST for SF 330s). TELEPHONE INQUIRIES WILL NOT BE RETURNED.
