Description
Underground Fuel Storage Tank Removal and Remediation Project #: 660-22-116 1. Introduction The Contractor is required to prepare a Corrective Action Plan and Tank Removal documentation for a permanently closed leaking underground gasoline tank release and a temporarily closed underground diesel tank . The tanks are located at the Department of Veterans Affairs (VA) Salt Lake City Medical Center. The primary object of the SOW is to safely remove, dispose of and remediate underground storage tanks (UST) while minimizing environmental impacts and complying with all Federal, State and local governing regulations, as well as VA policies. 2. Site Background and Location The work will be conducted at the Salt Lake City VA Medical Center (SLC VAMC). The SLC VAMC is registered as Facility ID # 4001459 with the Utah Department of Environmental Quality, Division of Environmental Response and Remediation (DEQ-DERR). Tank Descriptions and Status Tank 13(38G): 3,000-gal gasoline fiber glass UST; permanently closed in-place with cement slurry on 17-Dec-2020. Closure samples identified a fuel release associated with fuel piping near the dispenser island. A single closure soil sample exceeded the Initial Screening Levels for gasoline-range total petroleum hydrocarbons (TPH-GRO) (511 mg/kg) indicating a release may have occurred. The release was designated by DEQ-DERR as Release NZZ. The permanent tank Closure Notice is included in Attachment 1. A Subsurface Investigation Report (SIR) (Attachment 2) was completed in Jun/Jul 2021 and included drilling three additional borings around the potential release location and a fourth boring near the initial closure soil boring to greater depth (12 ft total depth). A total of four soil samples were collected from the four borings. The results indicated a minor, isolated fuel piping release had occurred near the dispensing island. Tank 12(38D): 3,000-gal diesel fiber glass UST; tank temporarily closed on 28-April-2021. Fuel was removed from tank, but no additional abatement procedures have been completed. No closure sampling has been completed at the tank. The temporary tank Closure Notice is included in Attachment 1. Fuel Piping and Dispenser Island: Fuel piping extends approximately 40 feet from underground tanks to dispenser island. The island contains two fuel dispensers. 4. Work to Be Performed The Contractor is required to complete the following tasks in accordance with Utah Administrative Code (UAC) Title R311-200 through R311-211 Environmental Quality, Environmental Response and Remediation, all other Federal, State and local requirements, and Section 02 65 00 Underground Storage Tank Removal of the VA Master Construction Specifications Index (Attachment 3). In the event UAC R311 contradicts VA Section 02 65 00, the discrepancy will be brought forth to the COR and a decision will be made which guidance to follow. a. Corrective Action Plan: Contractor will complete all work and documentation necessary to submit a Corrective Action Plan, prepared by a Utah-certified UST Consultant, with the presumptive remedy of excavation of contaminated soils. b. UST Permanent Closure Plan: Contractor will complete all work and documentation necessary to permanently close diesel Tank 12(38D). This Closure Plan will be submitted as an Appendix to the Corrective Action Plan. Soil samples collected as part of the tank closure procedure will be submitted to the analytical laboratory with a 24-hour turn-around-time to facilitate timely decisions regarding additional excavation/remediation. c. Pre-Removal Meeting: Contractor will conduct a pre removal meeting at the project site a minimum of 30 days before beginning work. Contractor will prepare the meeting agenda and minutes. The COR, VA SME/PM, and Utah-certified UST Consultant and Utah-certified UST Remover will be in attendance. d. Remove and Replace Site Structures: To facilitate the excavation of contaminated soils, the Contractor will remove pavement, carport, gate, fencing and other structures, as needed and replace the items when site work is completed. Existing materials (including clean overburden soil) shall be reused where possible. e. UST Excavation, Demolition and Removal: Contractor will complete all work and documentation necessary to remove Tank 12(38D) and Tank 13(38G) from the site and dispose of (or recycle) material off-site. Removal of tanks will include fuel piping and dispenser island excavation and removal. A Utah-certified UST Remover must remove the tanks, and a Utah-certified UST Soil and Groundwater Sampler will collect samples required for tank closure and disposal facility waste profile. All environmental samples will be submitted to a Utah-certified analytical laboratory. f. Excavation of Contaminated Soils: Contractor will excavate, contain, and dispose of contaminated soils at an off-site location. A portable volatile organic compound (VOC) monitor with a photoionization detector (PID) shall be used for soil headspace screening to assess excavation progress. The base price for the volume of soil excavated will not exceed 220 cubic yards (cyd) (or approximately 330 tons). Work beyond this amount will be based on a unit basis (per ton). g. Site Closure Sampling: Contractor will collect site closure samples from the excavation including excavation sidewall and bottom sample locations. The base price shall include 12 sample locations. Any additional sample locations will be based on unit pricing. Samples will be submitted to the analytical laboratory with a 24-hour turn-around-time. h. Site Restoration: Contractor will restore the site to existing conditions including backfilling, compacting and asphalt resurfacing. 5. Applicable Specifications and Requirements a. The Contractor will comply with the SLC VAMC Stormwater Management Plan (Attachment 4) and applicable Utah Construction Storm Water Pollution Prevention Plan (SWPPP) general permit requirements. b. The Contractor will clear all utilities and comply with VAMC Policy 00S.60 Excavation Safety Program prior to starting work (Attachment 5). c. The Contractor will submit all safety data sheets for any chemicals brought on-site and follow all safety requirements for preventing access to the work site. d. The Contractor will conduct work in accordance with the Section 02 65 00 Underground Storage Tank Removal of the VA Master Construction Specifications Index (Attachment 3) 6. Documentation Deliverables The Contractor will comply with all Federal, State and local governing regulations when preparing the documentation for this project. a. Draft and Final Corrective Action Plan (CAP). The Contractor will develop a CAP to address Release NZZ. The presumptive remedy is soil excavation and disposal. The Contractor will submit an electronic version of the draft CAP for review to SLC VAMC c/o CERCLA Program. The Contractor will address VA comments on the draft CAP and submit a final CAP to the VA. The VA will submit the report to the State. The final CAP submittal will include a hardcopy and electronic version. The electronic submittal will include all native files used to create the document. b. Draft and Final Permanent Tank Closure Notice Tank 12(38D). The Contractor will prepare a Permanent Tank Closure Notice for Tank 12(38D). The tank Closure Notice will be submitted to DEQ-DERR for tank closure and included as an appendix to the CAP for Release NZZ. c. Draft and Final Corrective Action Report for Release NZZ. The Contractor will prepare a Corrective Action Report documenting the successful remedial action and results of the site closure samples. The Corrective Action Report must include: - Narrative summary of methodologies, remedial activities and results, including documentation of personnel conducting site activities, timeline of events, and summary of results. - Figures documenting site features, excavation details, and sample locations. - Tabulated field screening PID results and analytical sample results. - Analytical laboratory reports, chain-of-custody, and soil backfill compaction documentation. - Photographs of tanks, excavation activities, and sampling. The Contractor will submit an electronic version of the draft Corrective Action Report for review to SLC VAMC c/o CERCLA Program. The Contractor will address VA comments on the draft Corrective Action Report and submit a final report to the VA. The VA will submit the report to the State. The final Corrective Action Report submittal will include a hardcopy and electronic version. The electronic submittal will include all native files used to create the document. d. Tank Materials and Soil Final Disposition Documentation. The Contractor will include in the Corrective Action Report the final disposition records and waste manifests for the underground storage tanks and appurtenances, as well as the soil excavated from the site. e. Period of Performance: 30 Days from NTP (Notice to Proceed) 7. Supplemental Documentation a. The Contractor will provide with their proposal, documentation that a Utah-certified UST Consultant, Utah-certified UST Remover and Utah-certified UST Soil and Groundwater Sampler, and Utah-certified analytical laboratory will perform the tasks described in the PWS. b. The Contractor is required to comply with all Federal, State and local regulations, policies and procedures regarding UST and leaking UST (LUST) . The Contractor shall comply with all changes to such Federal, State and local regulations and procedures which occur during the term of this contract.  The Contractor shall provide to the Government any additional certifications that may be required because of changes in such laws.  c. The Contractor will provide a contract liaison (and alternate) to coordinate work at SLC VAMC. 8. Other Conditions and Stipulations a. Contractor must provide written confirmation they have adequate insurance protection against negligent work and other forms of liability.  The nature and level of protection must be acceptable to the VA CO. b. The Contracting Officer s Representative (COR), Safety Officer, CERCLA Program Manager (or designated alternate) and VA Police have the authority under this contract to present a verbal stop work order pertaining to job activity they reasonably believe represents an imminent hazard to life, property, or the environment.  This verbal order shall be followed up by a written stop work order issued by the Contracting Officer as soon as feasible after the imminently hazardous situation has been stabilized or abated. VA will not incur additional costs if work is halted for good cause. c. The Contractor shall not be allowed use of government materials, equipment, and/or labor for completion of work under this contract. d. Provisions in this contract shall not constitute or operate as a waiver of any such provision, nor shall the failure to enforce any provision or of any other provision contained herein. If any provision of this contract shall for any reason be held to be invalid or unenforceable, such provision shall not in any way affect the validity or enforceability of any of the remaining provisions of this contract, and this contract shall be enforced as if such provision had not been contained herein.  9.
