Amendment 3 September 9, 2022 - 9:00 am Central Time � 2nd Site Visit All firms interested in attending the pre-proposal conference/site visit must submit the following information by email to Howard.Nesmith@fda.hhs.gov no later than 2:00 pm Central Time, September 8, 2022: � � �1.� Full name and telephone number of individual(s) attending; � � �2.� Company name, address, and telephone number; � � �3.� Certification of Vaccination Form. September 12, 2022 Question Due Date and October 11, 2022 Proposal Due Date remain unchanged. COVID-19 Information and Requirements As of August 22, 2022, to align with recently published CDC guidance, the Safer Federal Workforce Task Force identified changes to COVID-19 safety protocols for federal agencies. The FDA/HHS has instructed all operating divisions to implement the changes outlined below effective immediately. COVID-19 Vaccinations: The FDA is no longer collecting vaccination information from employees on a voluntary basis. Safety protocols for employees and contractors entering or working onsite at FDA facilities no longer vary based on an individual�s vaccination status. Information collected to date will be maintained in accordance with the National Archives and Records Administration�s records retention schedules.� COVID-19 Testing: The FDA is no longer requiring employees entering or working onsite at an FDA facility to participate in required COVID-19 screening testing regardless of their vaccination status. Employees working onsite may still request COVID-19 screening testing through the FDA-WTT on a voluntary basis, or when participating in an inspection where the firm is requiring proof of a negative COVID-19 test. Contractor and Visitor Requirements:�� The FDA is no longer requiring contractors and visitors to complete a Certification of Vaccination Form or have proof of a negative COVID-19 test when entering FDA facilities.��� HOWEVER, NCTR facilities are located within Jefferson County, of which COVID-19 rating is currently HIGH/RED STATUS. Therefore, before entering NCTR facilities proof of a negative COVID-19 test documentation as provided on the attached Certification of Vaccination and FACE COVERING IS REQUIRE. No Solicitation language will be made at this time given COVID-19 Guidelines are very fluid and subject to change at any time. At time of award the most current COVID-19 Guidelines will be incorporated into the contract. Attachment J-13 Certification of Vaccination for contractor and visitor certification of vaccination attestation at all FDA facilities. All on-site contractors and visitors will be required to have a completed Certification of Vaccination form on their person when entering a federal facility. If a contractor or visitor is not fully vaccinated, they must have in their possession documentation of a negative COVID-19 test result within the last 72 hours to show the Agency. All attendees will be required to sign in at the security gate and present a driver�s license or other government issued identification in exchange for a visitor�s identification card. Certification of Vaccination Form. It is MANDATORY to wear a well-fitting face covering/mask on campus while indoors, regardless of risk factors or vaccination status. If you are immunocompromised or at high risk for severe illness wear a face covering/mask that provides you with greater protection. COVID testing is required for all visitors that are not vaccinated or have not provided their vaccination information. Contractors and visitors must show a completed/signed Certification of Vaccination form (attached) or proof of a negative COVID-19 test result within the last 72 hours. All visitors are required to report to building #21 and present a valid government issued photo ID.� The visitor(s) must sign the visitor log book and be screened via the metal detector.� All the visitor�s possessions will also be screened.� Food items, video/audio tapes, computer equipment, disks and other magnetic media are not affected by the x-ray machine and will be scanned. The guards will then issue a visitor badge.� Visitors will turn in their visitor badge and sign-out on the log book upon exiting building #21. Access to the facility shall not be granted if the visitor(s) refuses to cooperate with these security measures. It is conceivable the Pre-proposal Conference/Site Visit will take several hours and cross over typical lunch hours. You are welcome to bring water for hydration. There are drink and snack vending machines located throughout the site and an on-site cafeteria featuring hot-line and grill. The closest off-site (gas station / food) is6 miles away.
