The associated NAICS code is 811219.� � v.�� �CLINS, item descriptions, quantities and units of measure:� 0001, Base Year, Annual service contract for X-ray insect irradiator , 09/26/2022-09/25/2023 1001, OY1, Annual service contract for X-ray insect irradiator , 09/26/2023 � 09/25/2024 2001, OY 2, Annual service contract for X-ray insect irradiator , 09/26/2024 � 09/25/2025 3001, OY 3, Annual service contract for X-ray insect irradiator , 09/26/2025 � 09/25/2026 � � �� 4001, OY4, Annual service contract for X-ray insect irradiator , �09/26/2026 � 09/25/2027�� � � vi.�� �Description/Statement of Work/Specification:� 1.�� �Annual service contract for Radsource X-ray insect irradiator (Model number: RS-2400Q). 2.�� �Service contract to include: a.�� �Unlimited repairs a.�� �Parts, travel and labor� b.�� �Response time within 24 hours after request c.�� �Turn around time within 1 week for issues that need to be resolved with a technician visit b.�� �Annual Preventative Maintenance (one per year) c.�� �Troubleshooting known problems including but not limited to: irradiation cycles not completing, vacuum not sealing properly, and voltage issues with arcing. d.�� �Replacement Guarantee in case the problem or repair cannot be addressed or fixed.� 3.�� �Preventative Maintenance includes but is not limited to: a.�� �Interior cleaning b.�� �Electrical & mechanical testing c.�� �Performance checks of all the components to ensure the instrument meets manufacturer performance specifications.� � � vii.�� �Date(s) and place(s) of delivery and acceptance:� Performance at: 64 Nowelo Street, Hilo, HI 96720 This requirement shall be FOB Destination.� � viii.�� �FAR Provision 52.212-1, Instructions to Offerors-Commercial, is incorporated by reference and applies to this acquisition.
