Request for Quotation (RFQ) W912LP-22-Q-4513 is for Occupational Health Medical Surveillance Examination services. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-07, effective 08/10/2022.�This procurement is being solicited for full and open competition. The NAICS Code is 62111 and the small business size standard is $12.0 million.� RFQ W912LP-22-Q-4513 is attached.� Submission Requirements: Offerors shall complete the SF1449, blocks 17a and 30a-c, and provide unit price and extended amount for each Item on the SF 1449 - Continuation Sheets (pages 3-6). Quotations shall be submitted via email to kelsey.l.letcher.civ@army.mil by the date and time indicated in this notice.
