- COMBINED SYNOPSIS/SOLICITATION FA487722Q0030 MEDICAL GAS DELIVERY Davis-Monthan AFB, AZ� 85707 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation FA487722Q0030 is hereby issued as a Request for Quotation (RFQ). This solicitation and the anticipated contract are designated for fiscal year 2023.� This solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular 2022-07, Defense FAR Supplement Publication Notice 2022-08-26, and Air Force Acquisition Circular 2022-07-01. The associated North American Industrial Classification Standard (NAICS) code for this request is 325120 with a size standard of 1000 employees. All prospective offerors must be registered in the System for Award Management (SAM) at www.sam.gov� and lack of SAM registration will make an Offeror ineligible for award.� The expected award type is a firm-fixed priced contract with one base year and four option years.� Offerors are advised that this is anticipated to be the one and only solicitation and discussions are not anticipated.� Therefore, offerors are encouraged to submit the best and final pricing for this requirement.� The right to make multiple or no award is reserved by the government.� �This solicitation is for medical gas delivery services.� Further details regarding the work to be performed by the awarded contractor are listed in the Statement of Work (SOW Attachment 1).� In accordance with the SOW, the Offeror shall provide a quote for the following services as outlined in the CLIN structure shown in the attached table (Attachment 3) The deliverables shall be sent electronically (i.e., email) to both the primary and alternate contract specialist below, and by the dates and times listed: Quotation � Pricing must follow the attached table (Attachment 3).
