Description
Eastern Kansas Statement of Work: Mobile CT Trailer The purpose of this document is to provide a statement of work (SOW), for the lease of CT Equipment listed below. This lease will be for VA Eastern Kansas (Topeka and Leavenworth campuses), for the term specified in this agreement. The CT will provide patient care coverage during the construction and installation phase of a newly procured Canon CT unit. The requested unit listed below meets the needs of the Eastern Kansas VAMC and aligns with the new unit being installed in terms of make, manufacturer and software configurations at both locations. Requirements (shall meet or exceed): The CT scanner shall be the Aquilion Fast Whole-body CT Scanner. The CT equipment shall include the AQUILION PRIME SP PLUS CONFIGURATION (80 ROW / 160 SLICE) WITH HIGH CAPACITY Medical Trailer Additional add-ons include: SECOND CONSOLE KIT FOR AQUILION PRIME ULTRA-FAST IMAGE RECONSTRUCTION KIT FOR AQUILION PRIME VERSION 7 UPGRADE FOR 1ST GEN PRIME DUAL CONSOLE 72 KW X-RAY HIGH VOLTAGE GENERATOR 600 MA UPGRADE ECG GATED SCANNING FOR AQUILION PRIME & LIGHTNING ECG MONITOR R WAVE CARDIAC TRIGGER 7800T VARIABLE HELICAL PITCH FOR AQONE (GEN 4 AND GREATER) WINDOWS 10 OPERATING SYSTEM PRIME FAMILY AND LIGHTNING 40-80-160 (PRE V6-REQUIRES SURECARDIO ECG GATING) HIGH-CAPACITY EXTENDED COUCH ACCESSORY KIT FOR HIGH-CAPACITY COUCH 7.5 MHU CT X-RAY TUBE CT PHANTOM 2 MULTIFUNCTION TASK CHAIR WITH ARMS 5 MEDIA, DVD-RAM DRIVE (9.4 GB) DICOM 3 STORAGE SERVICE CLASS PROVIDER (SCP) DICOM 3 MODALITY WORKLIST MANAGEMENT (MWM) SERVICE CLASS USER (SCU) SYSTEM DICOM 3 PERFORMED PROCEDURE STEP SCU DICOM 3 QUERY/RETRIEVE SCP DICOM 3 QUERY/RETRIEVE SCU AQ/MP DICOM COMMIT SCU PRESENTATION OF GROUPED PROCEDURES (PGP) AND EXAM HARD SPL BAYER MEDRAD INJECTOR ISI 900 Scope of Work: Delivery: The contractor will make delivery of the CT equipment F.O.B. destination point. Upon delivery, the Eastern Kansas VA Medical Center shall inspect the equipment for acceptance and will notify the contractor within 15 working days that the equipment has been received and accepted. If equipment is damaged or unacceptable for patient care, the equipment will be returned to the contractor for replacement. This will take place at both the Topeka and the Leavenworth campuses respectively. Contractor is responsible for Delivery Set Up to include: - Stairs with railing - ADA Handicapped Access - Straps/Tie down - Utilities VA Personnel will provide the plug for connection to the trailer - Outrigging/landing gear Provide information if available to stabilize the trailer Title to Equipment: The equipment will remain the property of the contractor. The Eastern Kansas VA Medical Center shall possession and use of the equipment shall not be disturbed in the event of bankruptcy of the contractor, corporate dissolution of the contractor, or other event, so long as Eastern Kansas VA Medical Center is not in default. The equipment shall remain in the possession of the Eastern Kansas VA Medical Center until expiration or earlier termination of this lease. Any assignment, sale, bankruptcy, or other transfer of the equipment by the contractor will not relieve the contractor of its obligations to the Eastern Kansas VA Medical Center, and will not change the Eastern Kansas VA Medical Center duties or increase the burdens or risks imposed on the Eastern Kansas VA Medical Center under this lease. Use of equipment: The equipment will be used only by the Eastern Kansas VA Medical Center in accordance with all applicable manuals and instructions, as provided by the contractor. The Eastern Kansas VA Medical Center will maintain the equipment in good repair, operating condition, and working order (ordinary wear and tear excepted), including but not limited to the performance of reprocessing, cleaning, and maintenance procedures described in the manufacturer s instruction manuals. Loss or damage of equipment. The contractor shall be responsible for all loss or damage to the equipment during periods of transportation and installation. During the time the equipment is in possession of the Eastern Kansas VA Medical Center, the Eastern Kansas VA Medical Center shall be responsible for any loss or damage due to the Eastern Kansas VA Medical Center fault or negligence. If the equipment or any component or accessory is lost while in Eastern Kansas VA Medical Center possession, the Eastern Kansas VA Medical Center will be liable for no greater than the fair market value of the lost item or items at the time of loss. The Eastern Kansas VA Medical Center shall assume risk of loss or damage to the equipment during relocation (i.e. moving the equipment from one Government location to another Government location) unless the contractor shall undertake such relocation. Invoices: The contractor will be provided a purchase order number from the Eastern Kansas VA Medical Center. The contractor must ensure the purchase order number is clearly identified on all invoices. The contractor will provide a monthly invoice. Fixed cost: The cost shall remain fixed for the term of the contract iaw Schedule B, unless adjusted as a result of a request for equipment modification from the VA. Warranty: The contractor will warrant and apply that the equipment will be fit and sufficient for the purpose intended as set forth in user manuals, merchantable, of good quality and free from defects in materials or workmanship, for a period covered by this lease. The contractor will warrant that it has good title to the equipment supplied and that the equipment is free and clear from all liens and encumbrances. If the equipment proves to be non-conforming with this warranty within such warranty periods, The Eastern Kansas VA Medical Center may return such equipment to the contractor for repair or replacement of the non-conforming or defective equipment. Repair or replacement of non-conforming or defective equipment shall be the contractor's obligation. The contractor will be responsible for costs associated with the return of non-conforming or defective equipment. Training: The contractor will provide on-site training for all VA personnel who perform routine care and maintenance to the equipment as described in the lease agreement. The contractor will provide operating, reprocessing, cleaning, and routine maintenance training for VA as described in the lease agreement. Maintenance: Lessor shall provide at its sole cost and expense: (a) Consultation for the purpose of advising Lessee of Site requirements for initiation of rental. (b) The Equipment as described in introduction. (c) Preventive maintenance and service on the scanner will be provided during the hours of 8:00 am and 9:00 pm, Monday through Friday, except when damage to the Equipment is as a result of the negligent acts or omissions of Lessee. Lessee will be solely responsible for reimbursing Lessor for expenses incurred by Lessor for the repair of such damage and for the labor costs of service performed outside covered hours. (d) Connection suitable for the 3-phase power source. (e) Applications training on CT Trailer no less than 4 operators Return of Equipment at expiration/termination of contract: The Eastern Kansas VA Medical Center at the expense of the contractor, shall immediately return the equipment, decontaminated, complete, and in good working order and condition (normal wear and tear excepted). Contractor will arrange for pick up, transport of the CT Trailer, and tear down. Period of Performance The commencement date of this lease will be determined based on the delivery of the new system in September 2022 and will run approximately 2 months or until completion of the newly procured CT installment project. The trailer will begin in at the Topeka campus (for approximately 4 weeks) and then moved to the Leavenworth campus (for approximately 4 weeks).
