Amendment 0001 - PWS, paragraph 1.3, ""60 days"" is corrected to read ""90 days."" See corrected attachment. Quotation due date/time remains unchanged. Request for Quotation (RFQ) W912LP-22-Q-4515 is for TDA Medical Equipment Annual Service. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-07, effective 08/10/2022.�This procurement is being solicited as a 100% small business set aside. The NAICS Code is 811219 and the small business size standard is $22.0 million.� RFQ W912LP-22-Q-4515 is attached.� Submission Requirements: Offerors shall complete the SF1449, blocks 17a and 30a-c, and provide unit price and extended amount for each Item on the SF 1449 - Continuation Sheet (page 3). Quotations shall be submitted via email to kelsey.l.letcher.civ@army.mil by the date and time indicated in this notice.
