Description
STATEMENT OF WORK (SOW) Tennessee Valley Healthcare System (TVHS) Contract Title: MPS 3 Myocardial Protection System Cleaning and Disinfection Service Contract Period of Performance: Base Year Timeframe: November 1, 2022 October 31, 2023 Background: The purpose of this contract is to establish a cleaning and disinfection agreement for two TVHS owned myocardial protection systems. TVHS currently owns two (2) Quest model MPS 3 Myocardial Protection Systems that require cleaning and disinfection on a periodic basis. This contract includes all labor, materials, disposables, chemicals/reagents, and travel required to complete the cleaning and disinfection of the MPS 3 units per original equipment manufacturer (OEM) guidelines. The specific terms and requirements for the required cleaning and disinfection services are detailed in this Statement of Work (SOW) and shall be met by the Contractor. Section 8. Equipment Listing details all equipment that shall be covered and maintained by the Contractor as part of this cleaning and disinfection contract. Weekly / 7-Day Protocol: The Contractor shall change the water in the circulation system of each MPS 3 unit on a weekly basis per section 1.3.5 of the Quest MPS 3 Cleaning and Disinfection Guide. Following the water change, the Contractor, per section 1.3.5 of the Quest MPS 3 Cleaning and Disinfection Guide, shall add Hydrogen Peroxide to the hypothermic reservoir and circulate to inhibit microbial growth. The Contractor shall drain water from the circulation system drain valve and set aside at least 5ml of water from the drain valve into a sterile sample container for testing hydrogen peroxide concentration. The Contractor shall ensure the hydrogen peroxide concentration level is at least 100 ppm, per OEM Guidelines. The Contractor shall complete surface cleaning of each MPS 3 unit on a weekly basis per section 1.1.1 of the Quest MPS 3 Cleaning and Disinfection Guide. Following completion of surface cleaning, the Contractor shall complete surface disinfection of each MPS 3 unit on a weekly basis with an approved disinfectant (CaviCide Spray and CaviWipes) per section 1.1.2 of the Quest MPS 3 Cleaning and Disinfection Guide. The Contractor shall complete system controller cleaning of each MPS 3 unit on a weekly basis per section 1.3.6 of the Quest MPS 3 Cleaning and Disinfection Guide. 28-Day Protocol: The Contractor shall complete the water circulation system cleaning procedure for each MPS 3 unit monthly per section 1.3 of the Quest MPS 3 Cleaning and Disinfection Guide. The Contractor shall prime the circulation system by ensuring the delivery set and accessories have been removed, the hypothermic reservoir has been drained and the water circuit adapter has been installed prior to proceeding with circulation system cleaning, per sections 1.3.1 and 1.3.2 of the Quest MPS 3 Cleaning and Disinfection Guide. The Contractor shall utilize Minncare HD disinfectant solution, acquired by the VA, for water circulation system cleaning (per section 1.3.2 of the Quest MPS 3 Cleaning and Disinfection Guide). After completion of circulation system cleaning, the Contractor shall first drain the system, then complete the water circulation system rinsing cycle (per section 1.3.3 of the Quest MPS 3 Cleaning and Disinfection Guide), and then shall refill the water circulation system (per section 1.3.4 of the Quest MPS 3 Cleaning and Disinfection Guide). The Contractor shall clean the surfaces of each MPS 3 unit using a lint-free cloth, soft-bristled brush, and CaviWipes with Minncare HD disinfectant solution that is compliant with section 1.1.1 of the Quest MPS 3 Cleaning and Disinfection Guide. The Contractor shall perform tubing change, every other 28-day period, including disinfection and cleaning of tubing. New tubing shall be provided by the Contractor. Hansen Fittings shall be reused once cleaned and disinfected. If Hansen Fittings are no longer in working condition, the Contractor shall contact the CO about the parts and/or repairs required. The Contractor shall complete delivery and vent valve cleaning for each MPS 3 unit every 28 days, per section 1.2 of the Quest MPS 3 Cleaning and Disinfection Guide. The Contractor shall visually inspect the valve mechanism for debris, and if possible, remove large debris prior to cleaning the valve mechanism. The Contractor shall use a 60cc syringe to saturate the inside and outside of the valve with warm water and use a lint-free swab to clean the inside of the valve above the plunger tip, per OEM Guidelines. Reporting Requirements The Contractor shall notify the Operating Room and Biomedical Engineering POCs (Points of Contact) of the exact date and time within three (3) days prior to performing cleaning and disinfection services. The Contractor shall be required to report to Biomedical Engineering to log in during normal working hours of 8:00 a.m. to 5:30 p.m., Monday through Friday. This check in is mandatory before any cleaning and disinfection service is conducted. Biomedical Engineering is found in building 1, room E01. Contractors are required to obtain a Temporary ID Badge from Engineering Services, room D73, while on VA property. After services are complete, the FSE shall document services and provide a copy of documentation to Biomedical Engineering and the Operating Room. The Contractor shall maintain a complete and chronological file of all cleaning and disinfection services completed, including a hard copy and electronic copy of weekly water change maintenance logs and 28-day protocol maintenance logs, for each MPS 3 unit owned by TVHS. Terms and Conditions: The Contractor shall perform cleaning and disinfection services on the dates provided in the schedule in the appendix. Up to 2 dates may be rescheduled at the Contractor s discretion with prior notification to the COR and/or Biomedical Engineering. Work must be performed and completed within the hours of 8:00 am to 5:30 pm, Monday through Friday, unless prior approval is provided by the COR and/or Biomedical Engineering. For any work to be performed outside of these hours, arrangements must be made with the COR and/or Biomedical Engineering to provide an escort. Contractor personnel completing cleaning and disinfection shall be trained by OEM on the MPS 3 system (cleaning and disinfection procedures). Any personnel performing cleaning and disinfection services shall provide documentation that they were trained by OEM (Quest Medical). The Contractor shall provide (as part of this cleaning and disinfection service contract) all cleaning/disinfection services, parts, labor, PPE, and all travel for each visit to TVHS. The VA shall provide Minncare HD disinfectant solutions and CaviWipes. If additional services, parts, labor and/or travel are necessary outside the scope and pricing of the original contract, the Contractor shall contact the CO before completing such work. The Contractor shall be liable for any additional services, parts, labor and/or travel (including labor hours) provided that are outside the scope and pricing of the original contract unless 1) the CO has been contacted about the additional parts and/or repairs required and 2) a corresponding contract modification has been put in place to cover and account for the additional parts and/or work required. Only those services specified within the contract are authorized. Before performing any service of a non-contract nature, the Contracting Officer or his/her designee must be advised of the reason for the additional work. If appropriate, the Contracting Officer or his/her designee may authorize the additional services under a separate purchase authorization. The Contractor is cautioned that only the Contracting Officer or his/her designee may authorize additional services and reimbursement will not be made unless this prior authorization is obtained. All work shall be performed in a professional manner by an authorized service representative. If any deficiencies are found due to negligence of the service representative, the Contractor shall be required to correct the deficiency to a fully operational status in accordance with manufacturer specifications at no additional cost to the Veterans Affairs (VA). The FSE hired for this work shall be an authorized FSE (no exceptions). The Contractor shall be able to read, write, speak, and understand English. All contractor representatives shall be sufficiently fluent in the English language to effectively communicate with TVHS representatives. The Contractor shall present a neat appearance commensurate with that required of a professional. Contractor personnel shall comply with medical treatment facility policies regarding personal appearance and conduct. All correspondence, service reports, and invoicing shall be in English and clearly legible.
