- VA BOSTON HEALTHCARE SYSTEM This notice is being published in accordance with Federal Acquisition Regulation (FAR) Part 5.101(a)(2) requiring the dissemination of information on proposed contract actions. This is a notice of intent to award a sole-source contract pursuant to 10 U.S.C 2304 (C)(1), as implemented by FAR 6.302-1(a)(2) and 6.301-1(c), only one responsible source and no other supplies od services will satisfy agency requirements, and brand-name descriptions. The Department of Veterans Affairs located at the 1400 VFW Parkway, West Roxbury Mass has a requirement for a 4-year Service Contract for the service described below. The Government intends to award a firm-fixed-price sole source contract. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. this notice of intent is not a request for competitive proposals. This is a written notice to inform the public of the Governments intent to award on a sole source basis. Interested parties may identify their interest and capability to respond to the requirement. Any response to this notice must show clear and convincing evidence that competition will be advantageous to the Government. This notice will close on October 10, 2022, at 2:00 Eastern Standard Time (EST PERFORMANCE WORK STATEMENT / SCOPE OF WORK Maintenance Repairs and Support on Accuray Cyberknife FIM 1.0 OBJECTIVE: To provide maintenance, repairs and support on the Accuray Cyberknife FIM radiotherapy system serial # C0536 located at VA Boston Healthcare System, 150 South Huntington Ave, Jamaica Plain, MA 02130, Radiation Oncology Department 2. 0 BACKGROUND: The VA Boston Healthcare System purchased an Accuray Cyberknife FIM system and the last day of the warranty is October 13, 2022. It is used to provide radiation therapy to Veterans to treat cancer. 3.0 SCOPE: The Vendor shall provide all labor, personnel, equipment, tools, materials, parts, supervision, and other items necessary to perform onsite and remote maintenance, repairs, and support on Accuray Cyberknife FIM system located in VA Boston Healthcare System, Jamaica Plain campus. 3.1 VA OWNED EQUIPMENT AND SYSTEMS INTEGRATED AND COVERED: Accuray Cyberknife FIM System serial # C0536 / EE# 206797 including the multi-leaf collimator, Cyberknife Optical & X-Ray Imaging and the Precision Treatment Planning System with additional TPS Workstations 4.0 DELIVERABLES: The following lists the deliverables that are expected to be provided through this agreement Manufacturer- required planned / preventative maintenance as scheduled during a mutually agreeable time with VA Boston HCS Corrective maintenance when requested by VA Boston HCS Software updates Clinical applications training Equipment diagnostic services via remote access 5.0 WORK PERFORMANCE: The main contact number for Clinical Engineering is (857) 203-5517. All contractor personnel, without exception, must report to the Clinical Engineering Department at VA Medical Center, Jamaica Plain campus, building 1, room 2B50, to sign in and to verify that they received a visitor identification badge before any work is performed. Contractor personnel will be escorted by Clinical Engineering staff to the work location and must return to Clinical Engineering to sign out and turn-in said badge along with the documented service report for the service performed. If there is a need to replace any hard drive, hard drives, and all information on them, are the property of the VA and must remain in the control of the VA Clinical Engineering department for proper destruction. 5.1 FIELD SERVICE REPORTS (FSR): The installation and testing of any On-site Parts shall be performed by vendor trained personnel. In addition, each FSR must, at a minimum, document the following data legibly and in complete detail: Name of contractor and contract number. Name of FSE who performed services. Contractor service ESR number/log number. Date, time (starting and ending), equipment downtime and hours on-site for service call. Identification of equipment to be serviced: Inventory ID number, Manufacturer's name, Device name, Model number, Serial number, Any other manufacturer's identification numbers. 6. Itemized Description of Service Performed (including, if applicable, costs associated with after normal working hour services) including: Labor and Travel, Parts (with part numbers), Materials and Circuit Location of problem/corrective action. 7. Signatures: FSE performing services described Authorized VA Employee who witnessed service described.
