CONTINUATION PAGE Statement of Work Transport and Disposal of Waste Pharmaceuticals BACKGROUND: Department of Veterans Affairs, Gulf Coast Veteran s Health Care System (GCVHCS) is seeking removal services of waste pharmaceutical products (hazardous and non-hazardous) and associated contaminated materials for 12 months plus 4 option years for the main campuses, Joint Ambulatory Care Centers, associated Outpatient Clinics, and Community Based Outpatient Clinics (CBOC). SCOPE: The contractor shall provide all labor and materials necessary for the collection and removal of waste pharmaceutical products (hazardous and non-hazardous) and associated contaminated materials for the Department of Veterans Affairs; Gulf Coast Veteran s Health Care System (GCVHCS). This contract includes disposal of used RxDestroyer containers. This contract does not include any pharmaceuticals that are placed in recycling or reverse distribution programs by the facility. This contract does not include any controlled narcotics that have not been treated in the RxDestroyer containers. Contractor shall provide clean replacement containers as needed on or before scheduled service days. Provided containers shall appropriate containers for initial accumulation of waste pharmaceuticals and overpack of expired pharmaceuticals from the Pharmacies. Accumulation containers will be black in color for hazardous waste pharmaceuticals and white with blue lids for non-regulated pharmaceuticals. Openings on containers will include industry standard retrieval prevention. Containers will be approximately 3 gallons in size or 7 gallons in size. Exact sizing must accommodate storage space in accumulation locations. Overpack containers must conform to DOT specifications for the waste being transported. Overpack containers will consist of 30 gallon plastic open top drums with secure lids unless otherwise approved by the Contracting Officer. Ensure provided containers are leak-proof, lidded, labeled to display type of waste and all other US DOT and state required marking to ensure compliance with all accumulation requirements for waste pharmaceuticals in 40 CFR. All containers shall always be damage free and in good working order. Pick Up Location: The COR is responsible to provide an initial drawing for GCVHCS and each CBOC and Outpatient Clinic identifying the correct location of pick up locations. This shall be updated quarterly to ensure it is always as accurate as possible. QUALITY CONTROL: Contractor must replace all containers that are more than approximately one-half (1/2) full, to ensure no overfills between service days. Contractor should have service infrastructure to handle all applicable issues related to waste disposal and service. Contractor should be able to complete all terms and conditions of the contract immediately following the award of the contract. There are to be no delays in delivering the service and/or products. CONTRACTOR S RESPONSIBILITY: Contractor will only transport the waste to facilities that are permitted for the processing of each waste. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to their employees, or others, as well as for any damages to personal or public property that occurs during performance of this contract that is caused by their employees' fault or negligence. 13. QUALITY ASSURANCE: The Government will investigate complaints from any facility. All information received will be provided either verbally and/or in writing to the contractor for appropriate action. 14. WORK HOURS: All pickups shall be performed between 8:30am and 3:30pm, Monday Friday or as established by the Facility COR/Point of Contact 15.
