- The United States Department of Agriculture (USDA) Animal Plant Health Inspection Service (APHIS) Plant Protection and Quarantine (PPQ) Phoenix Rearing Facility (PRF) �in Phoenix, AZ intends to award a sole source Purchase �Order to �Foss Therapy Services, Inc/SAM ID: TADAL13NTM57/CAGE CODE�� 412D3 for Irradiator Door Hinge Replacement of a Foss Therapy 484F-C in accordance with the statement of work.�� Pricing shall include all costs necessary to accomplish the services in the statement of work (including but not limited to: overhead, general and administrative, profit, insurance, transportation, labor hours, equipment and supplies)� This is a sensitive asset and the work involves shielding. This work must be completed by January 10, 2023. The rationale for not openly soliciting this requirement is In accordance with FAR 13.106-1(b)(1)(i) �(b) Soliciting from a single source. (1) For purchases not exceeding the simplified acquisition threshold. (i) Contracting officers may solicit from one source if the contracting officer determines that the circumstances of the contract action deem only one source reasonably available (e.g., urgency, exclusive licensing agreements, brand-name or industrial mobilization). Foss Therapy is the original manufacturer for� irradiator model 484F-C.� Since the work involves irradiator shielding, the vendor must be authorized by The Nuclear Regulatory Commission (NRC)� and the California Radiologic Health Branch (CRHB) �to work on that aspect of the machine.� Foss Therapy is �the only company authorized to work on their own machines in this context.� �This work must be completed as soon as possible to avoid the door sag causing the door to jam, potentially open, and thus render the unit inoperable and halting the entire Navel Orangeworm (NOW) sterile insect technique program. This Notice of Intent is not a request for competitive quotes. �No solicitation documents are available and telephone requests will not be honored.� However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as three (3) references from other customers who are using these products or services, to the Contracting Officer by 4:00pm. Mountain Time (MT) �on the closing date of this notice.� �Supporting evidence must be furnished in sufficient specifications and �detail to demonstrate the ability to comply with the above requirement.� The North American Industry Classification System (NAICS) code for this acquisition is 811219� Other Electronic and Precision Equipment Repair and Maintenance, and the �Small business size standard is $ 22Mil.
