Description
Department of Veterans Affairs, Albany Stratton VA Medical Center, Network Contracting Office 2, is issuing this Notice of Intent to Sole Source in order to identify any potential sources that have the resources and capabilities to provide a full preventive maintenance, repair and software updates for two Vision RT Align RT Systems at Albany Stratton VA Medical Center and Bronx James J. Peters VA Medical Center. SERVICES REQUIRED: Contractor shall provide annual service for preventive maintenance, repair and software upgrade for one Vision RT Align RT System at the Albany Stratton VA Medical Center, 113 Holland Ave, Albany NY 12208 and one Vision RT Align RT System at the James J. Peters VA Medical Center, 130 West Kingsbridge Road, Bronx, NY 10468 during the term of December 01, 2022, through November 30, 2023, with provision for four (4) Option Years. Contractor shall provide services for the Vision RT Align RT Systems. INTERESTED & CAPABLE RESPONSES: NCO 2 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services. Please provide all of the following in your response: Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address), GSA Contract number if applicable, and DUNS number. Please also include the North American Industry Classification System (NAICS) Code for which you propose to provide the service under for verification of your size standard. A Capability Statement which provides detailed, specific information that demonstrates understanding of the tasks required and capability to perform the tasks as stated in the attached Statement of Work. Failure to tailor capability statement to this specific requirement may result in your company being determined non-responsive. Responses must be submitted by 12:00 PM EST, Tuesday, November 08, 2022. Responses to the information requested above must be submitted via email to Levi.Russin@va.gov. This Special Notice of Intent to Sole Source is for planning purposes ONLY. The results of this notice may assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted for this notice. STATEMENT OF WORK (SOW) Comprehensive Service and Support of Vision RT Workstation System BACKGROUND: The Department of Veterans Affairs Albany Stratton and Bronx James J. Peters VA Medical Centers is seeking Full Preventive Maintenance, Emergency Repair, and Web-Based Technical Support services for its Align RT system made by Vision RT. The Align RT system tracks a patient s position before and during radiation therapy. This aids in setup and accuracy of cancer treatment. Contractor must perform all services required to complete the full preventive maintenance service of the systems. These services are necessary to the Medical Center s in order to provide accurate radiation cancer therapy and maintain patient safety. SCOPE OF WORK: The Contractor must provide all necessary labor, materials, travel, tools, equipment, transportation, and parts required to perform the Comprehensive Service and Support on the equipment listed in the SOW. Scheduled annual preventive maintenance must include electrical safety testing, lubrication, adjustment, calibration, testing, software updates, and replacement of parts during the performance of an Annual Preventive Maintenance service as required by Original Equipment Manufacturer (OEM) specification and processes found in the OEM s Preventive Maintenance Kit. All work must be performed by an OEM factory trained and certified technician. Upon conclusion of the maintenance performance, all equipment must be returned to operating condition compliant with OEM specifications. The Contractor must complete all required services within the month in which they are due. All parts must be new genuine OEM parts. The contractor must have ready access to these parts. The Contractor must provide access to web-based technical support as well as live toll-free technical support line during normal operating hours. The Contractor must provide access to all updated user manuals and references guides as released by the OEM. Services such as inspection, Original Equipment Manufacturer (OEM) required maintenance, or other maintenance and repair vital to the continued functioning of the systems may be added only after written modification from the contracting officer, unless they are inclusive to the requested services. No other person, including the COR or any other VA personnel are authorized to request additional work be performed by the contractor. Any additional services performed that are not authorized by the contracting officer will be considered inclusive.  REPORTING: The Contractor's service representative must contact the COR prior to any service performed. The Contractor must purchase Vendor Credentialling Service as part of physical security standards for the Albany Stratton VAMC. All contractor personnel must sign into the Medical Center upon arrival to provide service. All performance procedures must be provided in writing to the COR prior to the maintenance performed by the contractor. The contractor must inform the COR of any uncorrected deficiencies and note these on a service ticket. Notation must include the type of deficiency, dated and initialed. Any deficiency which poses a hazard to patients, staff, or other equipment must immediately be called to the attention of the users and the COR. CERTIFICATION: All work must be performed by OEM factory trained and certified technicians. The Contractor must provide certification as required by The Joint Commission (TJC) that all personnel are authorized to maintain the equipment specified by the contract and are competent and able to perform all duties listed under the terms of the contract. CONTRACTOR IDENTIFICATION: Contractor personnel must have in their possession for inspection by VA Police Department a valid state or government issued identification such as drivers license or passport. Personnel may be required to surrender their ID to the VA Police to obtain a temporary day pass at the discretion of VA Police. ID will be returned upon leaving US Government HIPAA COMPLIANCE: The Contractor must comply with all applicable HIPAA regulations and requirements necessary to protect the privacy, integrity & reliability of electronic Protected Health Information (ePHI) and provide proof of an existing Business Associate Agreement with the VA. This includes any exposures to ePHI by support personnel or disclosure of any ePHI to any other agents during the course of regular service and support activities, and to any ePHI obtained during the course of complaint investigation or any other FDA mandated activity. Under no conditions must any electronic storage media be removed from Veterans Affairs (VA) premises without being thoroughly sanitized to VA standards. Electronic media that cannot be sanitized to VA standards must be destroyed on-site by the VA The Contractor must provide any documentation required by law regarding the final disposal of any components and/or supplies, as defined by the EPA, the Resource Conservation & Recovery Act, and 6NYCRR, replaced during any service performed on the listed equipment. This documentation must include, but not be limited to, manifests, detailed lists of disposed waste, etc. showing cradle-to-grave documentation of proper disposal. PERIOD OF PERFORMANCE: The base period of performance will be from December 1, 2022, to November 30, 2023, with provision of four (4) Option Years.
