Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This is a Request for Quote (RFQ) and the solicitation number is 36C26223Q0090-01. The government anticipates awarding a firm-fixed price contract resulting from this combined synopsis/solicitation. This solicitation is issued as a 100% set aside for Service-Disabled Veteran Owned Small Business concerns. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at www.acquisition.gov. STATEMENT OF WORK (SOW) Same as SECTION D LIMITATIONS OF SUBCONTRACTING: The Limitations of Subcontracting will apply to this Solicitation per Federal Acquisition Regulation parts 19.505(a)(2) and 19.505(b)(1) as it pertains to subcontracting limitations. Vendors are also advised to familiarize themselves with 13 CFR 125.6 and the penalties described in this regulation. Include: As Primary Contractor: what % of Total Contract Value will you retain? If using a Subcontractor: what % of Total Contract Value will go to them? Quotes shall be submitted via email to Doreen.Masterson@va.gov and Offerors must reference Solicitation 36C26223Q0090-01 in the subject line of the email. No telephone request for information will be considered. Incomplete packages will be considered nonresponsive. All offers must be received by closing, no later than 3:00 PM MST, Monday, November 28, 2022. Prospective Offerors are notified that verification of VA Form 10091 and the W-9 sent directly to the paying office in Austin, TX, must be submitted with the offer unless the offeror has previously submitted these forms to VA Austin and provides certification of previous work. The FMS Vendor File Update form needs to be submitted in all cases to the Contracting Officer as part of the offer or proposal. Prior information sent to the VA Payment Center in Austin does not preclude submittal of the FMS Vendor File Update form. Failure to include this information could preclude the offeror from consideration for award or considerably delay award. Included: CEP Registration Guidance Venderization Copy of the SSN W-9 for TIN included in FAX to Austin VA Form 10091 (this gets faxed directly to Austin, TX see form instructions) FMS Vendor File Update Request Form
