- This amendment is to attach Solicitation Requirements. NO OTHER TERMS OR CONDITIONS CHANGE. 1. Scope: The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the vendor in supplying an Industrial Semi-Electric Pallet Jack Truck 3300lb Capacity, an Industrial Duty Pallet Jack Truck, 27""W x 48""L Forks, 5500lb Capacity, and a 128 Fully Powered Straddle Stacker, 3000lb Capacity 128"" for VA Loma Linda Healthcare System. 2. Background: VA Loma Linda Healthcare System has a purchase requirement for their New Offsite Warehouse. This will improve logistics and provide material handling equipment dedicated to the new warehouse for VA Loma Linda. The facility is seeking for a contractor to Global Industrial Semi-Electric pallet jack truck and Industrial duty pallet jack Brand Name or Equal, Big Joe S30-128 Fully Powered Straddle Stacker Brand name or Equal models, and quantities below: 1|Industrial Semi-Electric Pallet Jack Truck 3300lb Capacity Must lift and transport payloads up to 3300 pounds on rigid forks Must have automatic electro-magnetic brakes Must be standard 48 inches in fork length and 6 inches in width minimum Must hold minimum electrical charge of 3 plus hours per work cycle Powered by two (2) 12 Volt / 30AH batteries 1 Year Limited Warranty -100% parts and labor 1|Industrial Duty Pallet Jack Truck, 27""W x 48""L Forks, 5500lb Capacity Pallet size dimensions: A pallet jack must have minimum dimensions of 27"" W x 48"" L Load wheels must be a minimum of 3""; for Steer wheels: and height 7"".3"" lowered and 7.5"" raised Must have hold and move a minimum of 5500 Lb. Must be standard 48 inches in fork length and 6 inches in width minimum Must have 3 function hand level - Raise, Lower and Neutral 1 Year Limited Warranty -100% parts and labor 1|128 Fully Powered Straddle Stacker, 3000lb Capacity 128"" Must have a minimum of 3000 Lb. weight capacity at 24 load center Must have adjustable forks with range of 8 to 27 ½ inches minimum Must have Ergonomic operator handle for easy control Telescoping mast with minimum 128"" fork elevation Adjustable straddle legs with inside dimension (ID) that can be adjusted between 42 to 50 at no greater than 4 increments Must have electro-mechanical brake. Minimum of 4-hour continuous use per charge 1 Year Limited Warranty -100% parts and labor 3. Brand-Name or Equal Justification: VA Loma Linda Healthcare System has a New Offsite Warehouse. To meet the needs of the warehouse staff, dedicated material handling equipment in the new location is required. Equipment will meet specifications in Statement of Work. 4. Warranty of Equipment: Government requirement for standard manufacture industry warranty term. Government does not accept extended warranty, which requires advance payment. 4.a. Place of Delivery: 11201 Benton Street, Loma Linda, CA 92375-1000 4.b. Payment: Payment will be made upon receipt of a properly prepared invoice which references the period for which payment is due, the contract number and/or assigned purchase order number. The invoice must include the correct payment address. 4.c. All shipping costs will be included in the cost of the contract.
