OFFEROR READ THIS INFORMATION FIRST BEFORE SUBMITTING AN OFFER NOTICE TO OFFEROR: Due to the critical need for Hospitalist Physician Services at the Fargo ND VA Hospital, offeror should seriously consider their capability before submitting an offer. Inability to provide the required service will be annotated through Contract Discrepancy Reports and identified in the Government Contractor Performance Assessment Reporting Service (CPARS) which may affect future contract opportunities. Please ensure adequate and qualified candidates are available before submitting an offer. This a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is a solicitation conducted using FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26323Q0103 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-08 dated 10/28/2022. This solicitation is set-aside for Service-Disabled Veteran Owned Small Business (SDVOSB) entities. At the time of submission of offer and prior to award, the offeror shall be listed as verified in VIP database at https://www.vip.vetbiz.va.gov and verified and active as a Small Business in the System for Award Management (SAM) reflecting NAICS code 621111 Office of Physicians (except Mental Health Specialists) with a small business size standard of $14 Million. The PSC Code is Q201, Medical General Health Care. The VA anticipates the award of a Firm Fixed Price (FFP) contract for Hospitalist Physician Services. The contractor shall provide Hospitalist Physician (with experience) on site at the Fargo VA Hospital, ND 2101 N Elm Street, Fargo ND, 58102. See Performance Work Statement for specific requirements and qualifications. The period of performance is from January 16, 2023, to January 15, 2024 plus a twelve (12) month extension period. Solicitation Questions. All solicitation questions shall be submitted in writing via email to Pradeepkumar Elayath at Pradeepkumar.Elayath@va.gov. Questions must be received by 12/07/2022, 11:00 AM CST. Submission of Offer: Refer to FAR Provision 52.212-1 Addendum and 52.212-2 within the attached solicitation document for instructions on submitting an offer. Evaluation Factors. Lowest Price Technically Acceptable. Offerors shall submit required documentation outlined in FAR Contract Clause 52.212-2, Evaluation Factors. Offerors are responsible to review the solicitation s FAR provision 52.212-2 to determine the required documentation to be submitted with their offer. Offer Due Date: Quotes are due to the Contracting Office by December 10, 2022, at 11:00 AM Central Time. Quotes shall be emailed to Pradeepkumar.Elayath@va.gov prior to the solicitation closing date and time. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Officer prior to the solicitation closing date and time.
