Description
Question: Will the VA please provide a list of all medications that the VA Black Hills employed Eye Technician could administer to patients? Answer: Carboxymethylcellulose NA (pf) 0.5% 0.4ml- (2 boxes) Brimonidine tartrate 0.2% opth soln  (1 btl) Dorzolamide 22.3/timolol 6.8mg/ml opth soln  (1 btl) Fluorescein 0.25%/Benoxinate 0.4% 5ml (2btl) Latanoprost 0.005% (1 btl)  refrig Phenylephrine 2.5% opth soln 15ml  (2 btl) Pilocarpine 1% opth soln (1btl) Proparacaine 0.5% opth soln (2btl) Timolol 0.5% opth soln (1 btl) Tropicamide 1% opth soln 15ml (2 btl) Fluorescein opth Strip  (1 box) Tetracaine 0.5% dropperette (4 vial) Question: Is the 2 Nurse Practitioners, 3 RNs, 2 LPNs, and 2 MSAs listed VA staff and not contractor staff?  If it is VA staff, where is the VA anticipating this VA staff will be in the clinic using the minimum space requirements listed in the solicitation?   Answer: I listed all staff that are currently working out of Scottsbluff CBOC (contract and VA Staff) I will clarify: Contracted Staff: 2 NP s, 1 MD, 3 RNs, 2 LPN s, 2 MSA s VA Staff: 1 Pharmacist, 1 Eye Technician, 1 Social Worker Eye technician exam room is already listed in the Space Requirement, 1 Pharmacy office and 1 Social Work office will need to be added to the Space Requirements. Question: Does the VA prefer the contractor provide a copy of the TJC accreditation letter(s), certificate, and full report with the submission of proposal? Answer: Yes Question: If we elect to not perform routine x-ray services on site, can we send patients in need of these services to local Scottsbluff radiology groups (such as hospitals)? Answer: If it is chosen to not perform x-rays on site, yes a CITC consult can be placed for the patient to go to the community for x-rays. Question: In amendment 1 the government was asked to list all VA staff that will be onsite at the clinic. The VA answered with what appears to be all members of the clinic including contractor employees. Please clarify which employees the VA will staff at the clinic and which will be contractor employees. Answer: Yes, All staff were listed, contract and VA staff. Clarification of VA Staff Only: 1 Pharmacist, 1 Eye Technician, 1 Social Worker. Question: Please confirm transcripts are not required for candidates being submitted for this proposal. Answer: Provide a list of the names and titles of qualified physicians, physician assistants, nurse practitioners, nurses, Pharm-D, LICSW, Radiology Techs, and other primary care provider staff intended to be utilized in the performance of this contract, taking into consideration the enrollment numbers for the clinic and the minimum staffing requirements. Also provide the name and title of the Medical Director to oversee the clinic.
