- This not a request for quotes; the Government is seeking information for market research purposes only. The contractor shall provide Vertex DICOM CD/DVD Licenses. Place of Performances: West Texas VA Health Care System 300 Veterans Blvd Big Spring, TX 79720 San Angelo VA Clinic 4240 Southwest Blvd San Angelo, TX 76904 Abilene VA Clinic 3850 Ridgemont Dr Ste C Abilene, TX 79606 Odessa VA Clinic 8050 East Hwy 191 Odessa, TX 79762 Hobbs VA Clinic 1301 N Turner St Ste 420 Hobbs, NM 88340 The North American Industry Classification System (NAICS) is 334118- Computer Terminal and Other Computer Peripheral Equipment Manufacturing, Service Code (PSC) is 7F20- IT and Telecom - IT Management Tools/Products (Hardware and Perpetual License Software) Responses to this notice shall include company/individual name, size status for the above reference NAICS code, a service capability statement, SAM Entity Unique Identifier (EUI), DUNS Number, address, point of contact and examples of similar facilities to which similar services have been provided to. Contractors should provide information on whether they are certified SDVOSB, VOSB, Hub zone, 8(a), women-owned concern. If standard company brochures will be provided as a response to this Source Sought, ensure that additional information tailored to this notice is included. A solicitation is not currently available. If a solicitation is issued, it will be announced on the Contract Opportunities website https://sam.gov at a later date, and all interested parties must respond to that solicitation announcement separately from the response to this announcement. Contractors must be registered in System for Award Management (SAM), see internet site: https://www.sam.gov for information about registration. Specific areas to be addressed in capability packages submitted by respondents are as follows: 1. The vendor will deliver 5 of each: Vertex DICOM CD/DVD Licenses (Includes 1st year maintenance agreement per license) Connectivity fee for existing Epson CD burners DD On-Site Installation & Training (8 hours each day, 5 days total) Ink Jet full color set for Epson burner Specific Tasks: The contractor shall provide, install, and verify proper operation of the CD burner and associated viewing equipment and licensing for use by Health Information Management Service (HIMS) departments. The contractor shall set up and install the equipment as needed in each respective department to communicate with the facility s PACS, CPRS and VISTA, and shall allow processing, viewing, sharing, and burning of DICOM and JPEG images. The contractor shall provide a standard one-year warranty on all parts and labor starting at the approved completion of installation. This shall cover all updates to software and/or hardware, support, staff training, and shipping/handling. 2. Identify any other companies owned (wholly or in-part) by the owners of this business who provide goods or services that are registered under the same or a related NAICS code with the Center for Veterans Enterprise (CVE); Provide the certification type (SDVOSB/VOSB), Federal Identification Number, and state of incorporation for each. 3. Do you plan on responding to a solicitation for this requirement with a Joint Venture utilizing multiple owned companies as majority or non-majority owner? 4.
