- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-08 dated 28 October 2022. The associated North American Industrial Classification System (NAICS) code for this procurement is 624190. This solicitation number W91ZRS-23-R-0004 is for the Connecticut Army National Guard (CTARNG). The USPFO for Connecticut intends to award a Firm-Fixed Price contract for Employee Assistance Program Services. This award will be for employee assistance services encompassing counseling services, family care services, legal services, and financial services. Must meet all salient characteristics listed in the attached PWS. The Period of Performance will be 12 months from the date of award with no option years. This solicitation is for 100% Small Business set-aside. FOB Destination is Hartford, CT 06105. Instructions to Offerors 1. All questions shall be in writing. Questions may be emailed to james.e.turcotte3.civ@army.mil. Questions must be submitted no later than 08:00 a.m. EST, 22 December 2022. 2. All quotes are due no later than 08:00 a.m. EST, 29 December 2022. Quotes may be emailed to james.e.turcotte3.civ@army.mil. 3. Bid MUST be good for at least 30 calendar days after close of Buy. a. It is your responsibility to make sure your quote is received. The fact that the contracting office did not receive your quote due to an error on the email address does not change the due date or time. � Basis for Award Award shall be made to a single offeror. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The most important factors are price and past performance.� SAM Registration: Offeror must be registered in the System for Award Management (SAM) database before an award can be made to them. If the offeror is not registered in the SAM, they may do so through the SAM website at http://www.sam.gov. Payment shall be paid thru the wide area workflow program. All payments are net 30 days. All Offerors must accept this as payment. The wide area workflow website is https://wawf.eb.mil/. Offerors must include a list of their offer�s specifications along with their quote to determine technical acceptability. A small business joint venture offeror must submit, with its offer, the representation required in paragraph (c) of FAR solicitation provision 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services, and paragraph (c) of FAR solicitation provision 52.219-1, Small Business Program Representations, in accordance with 52.204-8(d) and 52.212-3(b) for the following categories: (A) Small business; (B) Service-disabled veteran-owned small business; (C) Women-owned small business (WOSB) under the WOSB Program; (D) Economically disadvantaged women-owned small business under the WOSB Program; or (E) Historically underutilized business zone small business. � Responses are due no later than 0800 EST, Thursday, 29 December 2022, and shall be electronically submitted to james.e.turcotte3.civ@army.mil. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34.
