- Statement of Work 1.0 General Intent 1.1 The contractor must provide all personnel, equipment, tools, materials, supervision, and other items and services necessary for the 18th Ave. Garage Siding Project located at 729 18th Ave, Menlo Park, CA 94025 1.2 The contractor must perform to the standards in this contract. This project shall take no longer than 60 calendar days from the notice to proceed to complete. 1.3 This project shall be supervised by the appointed COR to insure minimal impact to resident s day to day activities and minimal noise. Scheduling of work shall be at the convenience of the VAPAHCS. 2.0 General Information and Requirements 2.1 The objective of this project is to remove and replace the siding on the garage of 729 18th Ave. Menlo Park, CA 94025 to like new condition and up to CA residential code. 2.2 All work shall be coordinated with the COR assigned to this project and only commence once approved. 2.3 Badges: contractor will obtain required ID badges for all employees used on project at start of each workday; Report the COR for escort to the station police service for sign in and badge issuance. 2.4 Security: The contractor and their personnel shall be subject to the same Federal laws, regulations, standards, and VA policies as VA personnel regarding information and information system security. 2.5 Schedule: Normal work hours will be 8:00am 4:30 pm Monday-Friday excluding Government holidays unless otherwise approved by the COR. It is the intent of the Palo Alto VA Medical Center that the work is to be completed within 60 calendar days from the issuance of the Notice to Proceed (NTP). Any request to utility shutdowns must be requested in writing at a minimum of 30 days prior to the requested day of shutdown. 2.6 Codes and Standards: The Contractor shall follow all applicable codes and standards to this type of work including but not limited to Health Care System Memorandums (HCSM), OSHA Guidelines, Lock-out-Tag-out, and Rules of Station. If any code/standard is found to be contradictory; the authority having jurisdiction (COR) shall make the final determination and work shall proceed at no additional cost to the Government. 3.0 Specific Tasks 3.1 The Contractor to prepare and provide all supervision, administrative and technical support, labor, subcontractors, tools, transportation, materials, supplies and equipment to complete project. 3.2 The contractor will ensure all siding installation will be performed by a certified siding contractor. 3.3 The contractor will perform all work in accordance with all applicable codes, local ordinances, and requirements of NFPA and OSHA. 3.4 The contractor will demolish, remove, and replace house siding per finish siding specification while maintaining required fire rating. 3.5 Contractor to include all materials, siding, stainless steel screws and nails, as defined above in the total cost of repair. 3.6 The contractor shall utilize Hardi trim, and Hardi plank lap siding at 8 1/4 x 12 Cedarmill or equivalent systems. 3.7 The contractor shall provide and install prime & paint (2 finish coats) to the install siding system. Paint color shall be selected to match the existing dwelling on the property. 3.7 The contractor will provide (10) year warranty on all work performed. 3.8 The contractor shall remove and demo existing wood siding, trim, and moisture barriers of approximately 400 SQFT of siding surface. 3.9 The contractor shall assess for any dry rot and quote for any framing replacement required to replace found dry rot. The VA anticipates 5% of the total SQFT to result in dry rot. The VA shall review the proposal and reserves the right to accept the proposal or replace the dry rot under other means if required. 3.10 The contractor shall supply and install new Tyvek or similar building envelope system for moisture protection. 4.0 Technical Exhibits Specifications (see attached, listed below) SECTION 01 00 00 GENERAL REQUIREMENTS Section 01 33 23, Shop Drawings, Product Data, and Samples Section 01 35 26, Safety Requirements Section 01 45 00 Quality Control Section 01 74 19 Construction Waste Management Section 02 41 00, Demolition Section 07 24 00 Exterior Insulation and Finish Systems
