- Defense Health Agency (DHA), Southern Market Division Contracting Division, is issuing this Notice of Intent to Sole Source to identify any potential sources that have the resources and capabilities to provide Cepheid GeneXpert On-Site Preventative Service Maintenance Agreement in support of the 19th MDG located at Little Rock, Air Force Base (AFB). �The applicable North American Industrial Classification System (NAICS) code is 811210, Electronic and Precision Equipment Repair and Maintenance, Small Business Size standard is $34.0 million. Requirement: This non-personal services maintenance contract to provide On-Site Preventative Service Maintenance on a Cepheid GeneXpert (Serial Number: 846204) to 19th Medical Group located at 1090 Arnolad Dr. Little Rock, AR, 72099. The contractor shall provide preventative service maintenance to include all resources, labor, travel, transportation, parts, materials, equipment, and management necessary to keep the medical equipment in good operating condition and to perform all scheduled and unscheduled maintenance of the equipment listed in above. shall cover the repair, PM, calibration, loaners and exchange services according to the Manufacturer�s specifications for the equipment listed above Interested and Capabilities: DHA is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services.� Please respond with your companies: Business name Business type Socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.) Person of contact (including telephone number & email address) GSA Contract number if applicable SAM Unique Entity ID Capability statement Partnership Agreement Responses must be submitted by 1:00 PM (CST) 20 January 2022.� Responses to the information requested above may be submitted via email to Caitlin.e.hippensteel.civ@mail.mil.
