Description
DEPARTMENT OF VETERAN AFFAIRS LEBANON VA MEDICAL CENTER STATEMENT OF NEED (SON) Title: Oversized Wheelchair Scales Scope of Work: The Lebanon VA Medical Center (VAMC) has a need for seven (7) oversized wheelchair scales. The request is to replace several of the current wheelchair scales due to the inability to repair, as well as increase the number of scales suitable for the relocation of clinics throughout the facility and outpatient clinics. Salient Characteristics: The oversized wheelchair scale must meet the following: LCD digital display Battery operated Plug in AC power adapter Foldable platform for transport Footprint of the foldable platform must not exceed 24 inches wide for transport through interior doors Collapsible platform that locks in place for easy transport Pivoting wheels to move throughout the facility Platform must be between 32 34 wide without the ramps down Platform must be between 32 36 deep Platform must be no more than 52 wide with the ramps in place Scale must not exceed 3 from the floor to the top of the scale platform Two (2) roll-on ramps on each side of the scale Ramps must rest on the floor when in place for loading wheelchairs Weight capacity maximum of 1,000 pounds Must have pounds and kilogram measurement General Responsibilities: The contractor must provide all equipment, materials, documentation, and other items necessary to deliver and ensure proper functioning of all wheelchair scales purchased. Wheelchair scales must be delivered to the Lebanon VAMC fully calibrated and ready to use. Training and Installation: No training required. Warranty: All equipment and materials must come with a standard one-year warranty. Delivery: Contractor must provide delivery within 45 days of contract to the Lebanon VA Medical Center, located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn: Warehouse. Delivery to the facility must be coordinated at least 72 hours prior to required delivery date with the Warehouse Supervisor. Warehouse Supervisor: Phone: (717) 272-6621 x4077 Note: The Warehouse Supervisor has no authority to change the terms and conditions of the awarded contract. Work Hours: Standard Operational Hours are 0730-1600, Monday thru Friday. BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks.
