STATEMENT OF WORK (SOW) FOR OCTOPUS 900 VISUAL FIELD MACHINE VETERANS HEATH ADMINISTRATION 1. INTRODUCTION This solicitation is to provide the Veterans Health Administration, Wichita Advanced Low Vision Clinic with one (1) OCTOPUS 900 VISUAL FIELD MACHINE. This OCTOPUS 900 is needed to be able to provide visual field testing-including Goldman Perimetry and Esterman Binocular Perimetry for visually impaired veterans being seen in the Advanced Low Vision Clinic (ALVC). 2. BACKGROUND The Wichita VA ALVC, along with the Eye Clinic, currently use one (1) Humphrey visual field machine and one (1) Goldman Perimeter visual field machine. There are 4 FT Optometrist s and 1 FT Ophthalmologist that utilize these machines. The Eye Clinic utilizes the current Humphrey visual field machine for Glaucoma, Stroke, Macular Degeneration testing. ALVC utilizes the current Humphrey visual field machine for Esterman Binocular Perimetry testing for determining if a visually impaired veteran qualifies to drive. The Goldman Perimeter visual field machine is utilized primarily by the ALVC for testing visual fields in a simple manual operation. It is an older model and not automatic. The Goldman Perimeter machine only does manual Goldman testing for Glaucoma and Stroke. The visual field clinic, which uses the current Humphrey visual field machine, is frequently full and difficult to schedule last minute/same day when a veteran is determined to need a visual field test. This is not an effective way to provide standard of care testing for visually impaired veterans. The new OCTOPUS 900 VISUAL FIELD MACHINE will allow the ALVC and the Eye Clinic to provide more visual field testing as the OCTOPUS 900 will do Esterman Perimetry AND automatic Goldman Perimetry in the same machine. The ALVC would then be able to use this one OCTOPUS 900 machine and not need to utilize the current Humphrey visual field machine that the Eye Clinic uses, providing more testing opportunities for the Eye Clinic. 3. SCOPE OF WORK The contractor shall provide all personnel, equipment, tools, materials, supervision, and non-personnel services necessary to provide OCTOPUS 900 visual field cabling, accessories, programming the required software/hardware along with providing the necessary installation service for the OCTOPUS 900 visual field system. 4. TECHNICAL REQUIREMENTS A. Salient Features for OCTOPUS 900 visual field machine: * System includes: visual field machine, monitor, printer & wheelchair accessible table B. OCTOPUS 900 visual field machine Installation: * Set up, Install, Training provided by HAAG-STREIT USA * DICOM License P/N 7220374-1 5. DELIVERY & LOCATION INFORMATION Delivery of OCTOPUS 900 visual field machine will be shipped to: Robert J. Dole Medical Facility Building 34 NX Warehouse ATTN: Advanced Low Vision Clinic, Parklane Eye Annex 5500 East Kellogg Wichita Kansas, 67218 6. WARRANTIES: * Product warranty is standard 1 year. 7.Software/Hardware upgrades/Updates/Frequency * None are required, top of the line equipment 8.
