All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. The VA Maryland Health Care System is seeking to procure a service contract for the phototherapy units that are owned by the Baltimore VA Medical Center. The contractor shall perform annual preventative maintenance on the three units per the manufactures recommendations to assure optimum performance and reliability as well as the overall safety of the patients. The contractor must provide all the parts and services necessary for the maintenance and repair of the three provided phototherapy units. The equipment requires a service contract to ensure that the machines are properly maintained, are safe and functional for use. This contract should include 24/7 phone support, parts, travel, and labor, preventive maintenance, and upgrades included in service contract. STATEMENT OF WORK GENERAL INFORMATION Title of Project: Phototherapy Unit Support Scope of Work: The vendor will provide support for the three phototherapy units that are currently in use at the Baltimore VA Medical Center. Service coverage must include replacement parts, labor, travel expenses, tools, test equipment, and other incidentals necessary to perform scheduled annual preventive maintenance. Additionally, the contractor must provide technical and clinical application support by telephone, and remote monitoring and diagnostic support to minimize unexpected equipment downtime and maximize use of the equipment. The vendor must have an approved MOU/ISA with the Department of Veterans Affairs to provide remote support. The vendor will supply the Baltimore VAMC Biomedical Engineering department with a copy of a detailed service report that meets the requirements of 21CFR820, subpart N, within 2 weeks of maintenance being completed. The service shall be performed between the hours of 8:00am and 5:00pm, Monday through Friday, unless mutually agreed upon the contractor and the VA Biomedical Engineering department. The contractor shall schedule all maintenance with Biomedical Engineering and report to Biomedical Engineering in room 5B-183 upon arriving on-site at the Baltimore VAMC, before performing any maintenance. Below is the list of equipment identified to be covered by this service contract: Hand/Foot UVBNB SN 39443 Service Availability Monday Friday, 8am-5pm Thorough cleaning of the unit, inside and out Replacement of all lamps Removal and cleaning of reflector panels Training for staff on units as needed Next day repair parts if required for PM Uptime commitment of 95% Hand/Foot UVBNB SN 39442 Service Availability Monday Friday, 8am-5pm Thorough cleaning of the unit, inside and out Replacement of all lamps Removal and cleaning of reflector panels Training for staff on units as needed Next day repair parts if required for PM Uptime commitment of 95% Houva III UVBNB Phototherapy Unit SN 010300 Service Availability Monday Friday, 8am-5pm Thorough cleaning of the unit, inside and out Replacement of all lamps Removal and cleaning of reflector panels Training for staff on units as needed Next day repair parts if required for PM Uptime commitment of 95% Background: The VA Maryland Health Care System requires annual preventative maintenance coverage of the phototherapy units at the Baltimore VA Medical Center. This service coverage includes scheduled preventive maintenance in accordance with the manufacturer s recommendations to ensure that the equipment is properly maintained and to reduce downtime of the identified equipment that results in cancellations of patient appointments. Greene St Baltimore, MD 21201 Performance Based Contract: N/A Security: The contractor must have photo ID available when performing work at a VA Medical Center and be willing to present it to VA staff. The vendor is responsible for personal protective equipment.
