Combined Synopsis/Solicitation Notice This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Government reserves the right to make no award from this solicitation.   This is a request for quote (RFQ), and the solicitation number is 36C25023Q0636. The Government anticipates awarding a firm-fixed price Purchase Order resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circulars 2023-02 (eff. 03/16/2023). The associated NAICS Code is 339113, Surgical Appliance and Supplies Manufacturing.   The Department of Veterans Affairs, Network Contracting Office 10, is soliciting quotes from all sources to supply the Columbus VA Medical Center with Surgical Case Cart. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information, along with any other documents necessary to support the requirements below.   Quotes are to be provided to Kevin Peeples, Contracting Officer, via email at kevin.peeples@va.gov, no later than May 3, 2023, by 4:00 PM EST. No telephone calls will be accepted. No submission received after the above-mentioned date and time will be accepted.    STATEMENT OF WORK (SOW) 1. Contract Title Revolution Enclosed Surgical Case Cart 3. Background. The Chalmers P. Wylie Columbus VA needs 13 Surgical Case Carts, with wire shelves and solid doors that is Brand name or Equal to Pedigo Revolution Enclosed Case Cart. These Surgical Case Cart will be used at Chalmers P. Wylie Columbus VA Medical Center. Carts are used in Sterile Processing Service (SPS) as part of its function distributing sterile, high-level disinfection reusable medical devices used for direct patient care. It also transports contaminated reusable medical devices back to SPS for reprocessing. 4. Scope. Ability to transport reusable medical devices throughout medical facility in a safe manner preventing contamination. 5. Specific Tasks. Purchase Revolution Enclosed Surgical Case Carts as stated on quote for transporting reusable medical devices. The specs of the carts must be Enclosed Surgical Case Cart, windowed double door, (2) Total Lock Revolve casters and (2) Direction Lock Revolve casters, 36""W x 27-1/2""D x 59""H overall outside dimensions, Cool-Touch handles. Includes center mounted RCC-BH, Biohazard/Clean ID Wheel, (1) Biohazard label position, (1) Clean label position, (1) Neutral position flush to top. Brand name or Equal to the following. IMPORTANT: Please reference 36C25023Q0636 in the email 5.1 Task 1 - Deliverables: Brand name or Equal to the following Rev-o-lu-tion Enclosed Surgical Case Cart, windowed double door, (2) Total Lock Revolve casters and (2) Direction Lock Revolve casters, 36""W x 27-1/2""D x 59""H overall outside dimensions, Cool-Touch handles. Includes center mounted RCC-BH, Biohazard/Clean ID Wheel, (1) Biohazard label position, (1) Clean label position, (1) Neutral position flush to top. 13 RCC-235-B Roll-out wire shelf, for RCC-235 rev-o-lu-tion case cart 26 RCC-235-WSRO RCC-235 Solid Doors. Includes 2 solid doors instead of standard windowed doors. 13 RCC-235-SD Freight 1 SUBTOTAL (Products) 6. Performance Monitoring NA 7. Security Requirements No maintenance or installation of a VA Information System Hardware or Software is performed under this contract. No contractor access to a VA Information System, VA Protected Health Information (PHI), nor VA Personally Identifiable Information (PII) is allowed or performed under this contract. Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Not applicable to this request. 9. Other Pertinent Information or Special Considerations. Identification of Potential Conflicts of Interest (COI). There are no known conflicts of interest related to this request. c. Identification of Non-Disclosure Requirements. Not applicable to this request. d. Packaging, Packing and Shipping Instructions. Packaging will be removed from worksite by the award recipient. e. Inspection and Acceptance Criteria. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The award will be made to the response most advantageous to the Government. Responses should contain your best terms and conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."" OR ""The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"" Quoters shall list exception(s) and rationale for the exception(s), if any.
