Description
DESCRIPTION OF SERVICES: The South Texas Veterans Health Care System (STVHCS) requires continual maintenance and expansion of the facility. This current requirement serves the purpose of helping the lower level of the CLC located at Audie L Murphy in the conversion into a inpatient unit. The contractor shall provide all necessary labor, supervision equipment, machinery and tools required to complete the installation of TVs in the CLC. The Contractor shall provide all specific equipment needed to install blocking, TV�s, swing arms and boxes in order to turn the first floor of the CLC into an inpatient unit. �� 2.0 SERVICES & EQUIPMENT: 1. The Contractor will provide: a. All equipment and materials must be new, not refurbished, not rebuilt, nor 3rd party substitutes are acceptable. The contractor shall provide equipment to the following specifications: The TV shall be a 16"" LG hospital Grade TV TV shall be model 15LT766A0UA, 15LS766A0UA or equal. Swing arm shall weigh no more than 16.6 pounds, have a tilt degree of 15, have a display pan of 270 degrees, and a max extension of 67.8� and arm rotation of 180 degrees. Swing arm shall be cream in color and support 8 lb display devices. Swing arm shall be model number PDI-AA1408-CB or equal. Power mount shall be PDI-871-C or equal. Power mount shall be cream in color. voltage input shall e between 120-240VAC. Output shall be 24VDC at 2.5 amps Mounting Plate shall be model PDI-255E-C or equal. 3.0 GENERAL INFORMATION: 3.1 Hours of Operation Normal hours of operation are Monday through Friday from 8:00 am to 5:00 pm CST, excluding holidays. All services shall be performed during normal hours of coverage unless requested and approved by the COR. Federal and National Holidays observed by the VAMC are: National Holidays Federal Holidays: New Year's Day���������������� Labor Day Memorial Day������������������ Thanksgiving Day Christmas Day������������������ M L King's Birthday Columbus Day������������������ President's Day Veterans Day�������������������� Independence Day Also included would be any other day specifically declared by the President of the United States to be a National Holiday. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. 4.0 FACILITY POINT OF CONTACT: VA Contact Person: Abdias Villanea or Brittney Brown Contractor Telephone Number(s) of Service Department: 210 617 5300 ext. 15721/16612 5.0 COMPLIANCE WITH SECURITY & INFECTION CONTROL: 5.1 Identification: All contractors working shall be fingerprinted prior to being allowed to work on campus. Contractor ID badges can be obtained from PIV office typically 3-5 days after approved SAC form is received and fingerprints clear. ID badges are required for contractor employees working at Audie L. Murphy Medical Center. 5.2 Parking: It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VAMC will not validate or reimburse for parking violations from the Contractor under any conditions. 5.3 Smoking, Weapons, and VA Regulations: Smoking is prohibited inside any building at the VAMC. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable to the United States (Federal) District Court, not a local district, State, or Municipal court. 5.4 Vaccinations and Training: All contractors must show proof of NEGATIVE TB results, annual Flu vaccination and COVID vaccination. All contractors must attend Infection Control Training held every Tuesday at 10am at the Audie L. Murphy Memorial Hospital located at 7400 Merton Minter, San Antonio TX 78229. 6.0 Technical Requirements: The Contractor shall deliver the hardware, software and services necessary to provide installation of TVs on the first floor of the CLC at Audie L Murphy that meets the following requirements: a. The Contractor shall provide and install a 16� arm-mounted patient TV. b. Tv shall be 16�, have a resolution of 1920x1080 minimum and a backlight LED. c. TVs shall have forward facing speakers with a minimum of 2x1.7-Watt RMS and. direct channel entry buttons. d. TVs shall have a front headphone jack and the option for a pillow speaker. e. TVs shall have programable source selection, password protected menus, a maximum consumption of 25W. f. TVs shall weigh no more than 7 pounds and be compatible with a swing arm. g. Swing arm shall weigh no more than 21 pounds, have a tilt degree of 15, have a display pan of 270 degrees, and a max extension of 72� and arm rotation of 180 degrees. h. Central power supply shall have a conversion type of 1KVA transformer, be 89% efficient. i. Current for the power supply shall not exceed 10A and output shall not exceed 28/32 VAC configurable. j. The cabinet color shall be cream, able to be wall mounted, and weigh no more than 28 pounds. 7.0 Security: a. The contractor shall provide security for the vendor provided equipment. b. The Contractor shall provide for the physical security of Contractor equipment, as needed. VA shall provide the Contractor with escorted access to the wiring closets. c. Log files must be maintained for a one-year period. d. The Contractor shall adhere to all physical security and badge identification outlined by the VA Police while on site. �� 8.0 DEFINITIONS/ACRONYMS Acceptance Signature � VA employee who is authorized to sign-off on the ESR, which indicates that the PM has been concluded or is still pending completion, or that the Emergency Repair has been accomplished or is still in a pending status. Authorization Signature � the COR�s signature; indicates that the COR accepts work status as stated on the ESR. CO � Contracting Officer COR � Contracting Officer�s Representative VAMC � Department of Veterans Affairs Medical Center �� 9.0 Information Systems Officer, Information Protection: The contractor will not have access to VA Desktop computers, nor will they have access to online resources belonging to the government while conducting services. If removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. 10.0 Privacy Officer: The contractor will/will not have access to protected Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. All research data available for Contractor analyses is de-identified. 11.0 Records Manager: Records Manager: a. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. b. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. c. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. d. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. e. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. f. The Government Agency owns the rights to all data/records produced as part of this contract. g. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.
