- 1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures, specifically FAR 13.5 and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation is attached to this announcement. 1.A. Project Title: Mobile Heated Plate and Dish Dispenser. 1.B. Project Location: Southern Arizona Veterans Health Care Center (SAVAHCS) Nutrition and Food Service Located at 3601 S 6th Ave, Tucson, AZ 85723 1.C. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26223Q0915 1.D. Set Aside: This solicitation is 100% set aside for Service-Disabled Veteran Owned Small Businesses IAW Public Law 109-461, 38 USC Section 8127 1.E. Applicable NAICS code: 333241 1.F. Small Business Size Standard: 500 Employees 1.G. Type of Contract: Firm Fixed Price 1.H. Delivery: Delivery is expected 180 days after award. 2. Complete and submit enclosures 1 and 2 via email no later than 11:00 AM Friday May 12, 2023 to David Odne; david.odne@va.gov. Ensure the following are completed and/or included in the offer packet: 2.A. Vendor Information 2.B. Acknowledgement of amendments 2.C. Concurrence/non-concurrence with solicitation terms, conditions, and provisions 2.D. Schedule of services 2.E. FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items 2.F. 852.219-78 VA Notice of Limitations on Subcontracting Certificate of Compliance for Supplies and Products 3. Scope Southern Arizona Veterans Health Care Center (SAVAHCS) Nutrition and Food Service requires three (3) mobile heated dish and plate dispensers to replace the existing units that are beyond their life cycle. 3.1 Vendor shall provide three (3) mobile heated plate and dish dispensers according to listed Salient Characteristics below. As well as a minimum of a one-year warranty included in this requirement. 4. Salient Characteristics: - Unit(s) must be name-brand or equivalent in capabilities to the Lakeside model 6209 Mobile Plate and Dish Dispenser. - Unit(s) must be of stainless-steel construction - Unit(s) must have heavy duty casters at least 4 inches in diameter and with at least two casters having brakes - Unit(s) must have two dispenser tubes that are fully enclosed and can hold 9 inch plates with a vertical stacking capacity of 25 inches or more. - Unit(s) must have dispenser tubes that are self-leveling and with springs that can be adjusted to support different dispenser levels without use of tools - Unit(s) must have separate thermostat controls for each dispenser in a tamper-proof location with the thermostat control having variable settings to allow for temperature adjustments. - Unit(s) should have an easily accessible and visible power switch and power indicator light to ensure that staff can identify when the dispenser is on. - Unit(s) must be able to meet or exceed an interior temperature of 175 degrees Fahrenheit. - Unit(s) must be capable of being powered by 120V, single phase power. - Unit(s) must have external power cord that can be secured or designed to not touch the ground when removed from power source. 5. Packaging, Packing and Shipping Instructions. Packaging to include all user guides and manufacturer cleaning instructions. 6. Inspection and Acceptance Criteria. Equipment must be new and in good working order. Service line POC: Andrew McCrary will coordinate with food service staff to inspect and test equipment upon arrival. Equipment Inventory Manager: Carlos Meza will be contacted by Warehouse personnel to come and tag equipment upon arrival. 7. Contractor is to order and store at contractor s expense any long lead delivery items that can delay or impede the completion of this work. 8. Whenever contact with a facility is required outside of normal duty hours, the Contractor shall coordinate these times with the designated COR, POC or designee. 9. In accordance with 5 U.S.C. 6103, Executive Order 11582 and Public Law 94-97 the following national holidays are observed and for the purpose of this contract are defined as Legal Federal official holidays New Year s Day January 1st Martin Luther King s Birthday Third Monday in January President s Day Last Monday in January Memorial Day Last Monday in January Juneteenth June 19th Independence Day July 4th Labor Day First Monday in September Columbus Day Second Monday in October Veteran s Day November 11th Thanksgiving Day Fourth Thursday in November Christmas Day December 25th 10. Noncontract Charges: The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer. 11.Waste Removal: The contractor is responsible to remove and properly dispose of all waste related to the services provide on-site at all VA facilities. 12. Failure to carry out the above procedures will be considered as a failure to document the service visit and will constitute a failure to perform. 13. All shipping costs will be included in the cost of the contract. 14. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-04 Effective January 30, 2022.
