- 1 year maintenance agreement for Attune NxT Flow Cytometer (6/12/23 - 6/11/24). Also see attached Statement of Work. AB Assurance Plan 1. Parts, labor and travel for remedial repair. 2. No charge for planned maintenance visits. The annual planned maintenance (PM) visit is automatically opened and will be performed within the contract period.� 3. Guaranteed priority response time of 2 business days after receipt of a service call.� If contracotr fails to arrive at the instrument location within 2 business days for reasons other than customer's failure to provide access or causes beyond the reasonable control, contractor will provide customer a service plan renewal credit in an amount equivalent to one day's pro-rated charge for each day response is late. 4. Target response time of 3 business days for remedial repairs. Contractor will use reasonable efforts to respond within 3 business days from receipt of a service call. 5. Priority telephone and email access to instrument technical support. 6. Telephone and email access to application technical support. 7. Remote Monitoring and Dx Service, which provides for notification to customer of instrument failures or errors that are reported by AB's Remote Monitoring software. Inclusions 1. Repairs. In the event Customer's instrument requires repairs,Customer will send Customer's instrument covered by this plan to a Repair Center. Cost of parts and labor for remedial repair is included. Contracotr will use reasonable efforts to perform the repair and ship the Customer's instrument back to the Customer within 15 business days from the date of receipt of such instrument at a receiving centre or within a reasonable time thereafter. 2. Temperature Recalibration. Contractor will test and verify the temperature accuracy of the instrument and, if it is determined to be out of accuracy specifications; recalibrate the instrument's thermo block control. 3. Certification Performance Test Series. Contractor will test and verify that the instrument's heating and cooling rates, average cycle time standard deviation, and thermal non-uniformity are within published specifications. 4. Firmware upgrade. The instrument will be upgraded with the latest version of firmware, unless the customer directs in writing not to perform the upgrade. 5. Costs of shipping of both replacement and original instruments will be borne by the contractor. 6. Technical Support. Telephone access to instrument technical support. Exclusion Planned maintenance service. Description of the AB Rapid Exchange Service Plan The Rapid Exchange Service Plan provides a customer with a replacement unit of the same instrument model covered by this service plan should contractor determine that contractor cannot repair the customer's instrument through telephone support. The customer's instrument must be in good working condition at the time of purchase of this plan. Rapid Exchange Service Plan features include: 1. Guaranteed Shipment. Contractor will ship a factory-certified refurbished unit of the same instrument model covered by this service plan within one business day from the time that contractor determines that a replacement instrument is required. Customer may receive a new instrument if a refurbished instrument is not available. 2. Shipping Costs. Contractor will pay the costs of shipping the instrument being replaced from the customer's site to contractor's receiving center as well as the costs of shipping the replacement unit to the same customer's site. 3. Technical Support. Priority telephone and email access. Rapid Exchange Service Plan excludes: 1. Planned maintenance(PM) and remedial repairs and parts are not covered under this plan. 2. Computer hardware is not covered by this plan. Rapid Exchange Service Plan Process: 1. If the instrument requires repair that cannot be accomplished via phone support, contractor will initiate the shipment of a replacement unit. 2. Contractor will send customer a ""Returns Documentation Package"" via Fax or email. 3. Customer must follow the instructions on the return documentation for return shipment. 4. Customer must return the instrument via the carrier designated in the instructions, with a properly completed certificate of decontamination, using the packaging in which the replaccement instrument arrived. Customer must return the instrument being replaced within the period specified in the instructions.
