- Page 2 of 2 This is a combined synopsis/solicitation for Brand Name or Equal Commercial Items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 36C10G23R0006 is issued for this combined synopsis/solicitation; this solicitation is being issued as a Request for Proposal (RFP). This combined synopsis/solicitation incorporates provisions and clauses in effect Federal Acquisition Circular (FAC) 2022-06 dated 26 MAY 2022. Requirement Description: VA has a requirement to purchase Dental Delivery Units and Chairs devices to deliver patient care at its VA Medical Centers around the country. Delivery Units and Chairs are designed to facilitate dental examination, treatment, and/or minor surgical procedures performed in dental clinics. The Dental Delivery Units can be configured for rear or side delivery and integrates instrumentation mounts, cuspidors, assistant s holders, lights, a foot control and an adjustable electric chair adaptable to correct body positioning of the patient during the examination, treatment or cleaning of patients teeth. Dental Delivery Units and Chairs may be partially or completely attached as integral components or may stand separately as self-supported units. A single award Requirements contract will be awarded IAW FAR Clause 52.211-6, Brand Name or Equal and FAR 11.104, Use of Brand Name or Equal Purchase Descriptions, in accordance with all terms, conditions, provisions, specifications, and schedule of this solicitation herein. Proposals shall contain the terms for cost/price and technical capabilities of the band name or equal equipment. The Government reserves the right to award without discussions. The associated North American Industrial Classification System (NAICS) code for this procurement is 339114 Dental Equipment and Supplies Manufacturing, Product Service Code: 6520 Dental Instruments, Equipment and Supplies and the associated size standard is 750 employees. This procurement solicitation is issued as 100% Small Disable Veteran Owned Small Business (SDVOSB) Set-Aside. This acquisition is for a single-award Requirements contract with Firm Fixed Priced orders in accordance with FAR Part 16.5 for DentalEZ Forest 3900 and 4486PI Brand Name or Equal Dental Delivery Units and Chairs and related equipment and accessories, as identified in this solicitation. All interested companies shall provide a quote for all Contract Line-Item Numbers (CLINs) listed in ATTACHMENT A PRICE LIST- DENTAL DELIVERY UNITS AND CHAIRS. This procurement effort is to establish a 12-month base contract with four 12-month option periods. Delivery is FOB destination and requires delivery to VA medical facilities throughout the United States. Firm-Fixed-Price Orders will be placed against this contract in writing and will provide the exact delivery locations, delivery dates, and exact quantities. Delivery shall be provided within 30 days after receipt of order (ARO). The FAR provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition including attached addenda to the provision. The FAR provision at 52.212-2, Evaluation -- Commercial Items, and the specific evaluation criteria as attached addenda also applies to this acquisition. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer, if not available in the System for Award Management (SAM). Vendors are required to complete all fill-in Provisions listed on Attachment D - Solicitation Provisions. Clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition and a statement regarding any addendum to the clause. Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition and found in Attachment C - Contract Clauses. The FAR Provision found at 52.204-7(b)(1) requires that All interested Offerors must be registered in SAM prior to submitting an offer or quotation. BACKGROUND: The Veterans Health Administration (VHA) Non-Expendable Equipment National Program has identified Dental Delivery Units and Chairs product line as a candidate for a VA-wide (otherwise referred to as national ) single Requirements contract. Dental Delivery Units and Chairs are designed to facilitate dental examination, treatment, and/or minor surgical procedures performed in dental clinics. The Dental Delivery Units can be configured for rear or side delivery and integrates instrumentation mounts, cuspidors, assistant s holders, lights, a foot control and an adjustable electric chair adaptable to correct body positioning of the patient during the examination, treatment or cleaning of patients teeth. Dental delivery units and chairs may be partially or completely attached as integral components or may stand separately as self-supported units. The Dental Delivery Units and Chairs required for this program have been identified as most advantageous to the Government to be incorporated into a national Requirements contract. The salient characteristics (SC) for these commercial items are identified below. The Contractor must demonstrate the ability to meet all requirements for the solicitation. The objective is to provide Dental Delivery Units and Chairs to be used by clinicians throughout the VA medical centers and facilities. The period of performance is for one 12-month base period with four 12-month option periods. 2. REQUIREMENT This requirement will be conducted In Accordance With FAR 52.211-6, Brand Name or Equal which requires the offeror indicate that each product being offered as an equal product to the DentalEZ Forest 3900 and 4486PI Dental Delivery Units and Chairs. For each equivalent product proposed, the offeror must include a description reflecting the salient characteristics (SC) are met and level of quality that will satisfy the salient physical, functional, or performance characteristics of the brand product(s) specified in the solicitation. The SC table below shall be included in the beginning of the technical volume with a cross-reference column of the page number(s) identifying where in the proposal that the requirement is met. The offeror must clearly identify the item by brand name (if any) and make/model number. Finally, the offeror must include descriptive literature, such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer, and clearly describe any modifications it plans to make to a product to make to make it conform to the solicitation requirements. The Brand Name or Equal requirement applies to CLINs 0001-0002 only. The Government's intent for this Requirements contract is to include all ancillary items to ensure the equipment can function as designed by the OEM and as clinically required. The Government may through bilateral modification, add or remove items to this contract that are determined to be within scope of the solicited product line, not limited to the original items awarded. Additionally, any discontinued items may be replaced via future contract modification by the replacement part identified by the OEM, so long as the replacement part serves the same clinical purpose and function as the originally awarded item. Furthermore, any new technology that is presented during performance of this contract may be evaluated by the Government for inclusion into the contract, via bilateral modification. Vendors may propose any product solution or configuration so long as they meet the salient characteristics. Items proposed for CLIN 0007 CLIN 27 must be at a minimum compatible with the models found on CLINs 0001-0002. Vendors may propose any additional ancillary products that are the same or equivalent to CLIN 0007 - 0027 which they deem to be essential to the functionality of the proposed solution; these items must be captured in the vendor s pricing volume with an estimated quantity of one. Vendors may propose any additional ancillary products which they deem to be essential to the functionality of the proposed solution; these items must be captured in the Vendor s pricing volume with a quantity of zero.
