Description: Please send in the signed SF1449, QASP and memo stating that you can handle all that is described in the performance work statement. Please provide an unexpired and active SAM. The vendor shall provide outreach, a bed, 3-meals per day and case management services for 8-male veterans at the vendors VA approved site located on the Island of Oahu, Hawaii. Submission instructions can be found on pages 73 and 74 of the SF 1449. This program is to help out homeless or at risk veterans.
