- This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this sources sought must be in writing. The purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) Unique Entity ID (f) Cage Code (g) Tax ID Number (h) Type of small business, e.g. Services Disabled Veteran Owned small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business and (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The VA Loma Linda Healthcare System (VALLHCS) located at 11201 Benton St, Loma Linda, CA 92357 is seeking a potential qualified contractor that can replace carpet in its neurology suite. Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Business set aside. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 238330 ($19.0 Million Dollars). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov. Additionally, all Service Disabled Veteran Owned Businesses or Veteran Owned Businesses who respond to a solicitation on this project must be registered with the SBA Veteran Small Business Certification (VetCert) Registry located at https://veterans.certify.sba.gov. STATEMENT OF WORK TITLE: Neurology Suite Carpet BACKGROUND INFORMATION: VA Loma Linda Healthcare System (VALLHCS) Main Hospital is located at 11201 Benton Street, Loma Linda, CA 92357. The Neurology Suite located in 2 Southwest. The carpet flooring in the Neurology Suite is faded, old and dirty, with stains that are unable to come out after several attempts to clean by the Environmental Management Services (EMS) Department. The rooms that are included in the Neurology Suite are the common area, 2A-53A, 2A-54, 2A-55, 2A-57, 2A-58, and 2A-60. The carpet in these rooms need to be replaced. These rooms have been identified as having flooring was installed using asbestos containing materials (ACM). These rooms require asbestos abatement on the current flooring due to the presence of ACM. PART and EQUIPMENT LIST Part/Equipment Model/Part Number Serial Number Carpet Tiles Achromatic Color Black 5T1077 Cove Base Tarkett Color Pewter OBJECTIVES: The purpose of this contract is to have a contractor furnish all labor, materials, supplies, tools and qualified personnel to replace the carpet in the Neurology Suite on the 2nd floor, rooms 2A-53A, 2A-54, 2A-55, 2A-57, 2A-58, and 2A-60. Prior to the carpet installation, the vendor shall hire an independent outside contractor to perform abatement 660 square feet of asbestos containing materials (ACM) found in the black mastic that was used as adhesive in the old flooring and cove base. Abatement and carpet replacement shall include the following: Asbestos Abatement: The area shall be vacated. All furnishing and non-attached equipment to be removed by the Flooring contractor prior to the abatement. Set up a negative air machine with plastic containment and seal all entrances to the areas using an air scrubber HEPA filtered machine inside each room with a minimum of 4 room changes/hour before disturbing any affected areas. Run 120 feet of duct down the hallway to stairway #7 on the 2nd floor. Duct shall be suspended at least 8 feet off the floor so that it doesn t pose as a trip hazard. Connect the duct to the negative air machine. Abate approximately 660 square feet of carpet, Vinyl Composite Tiles (VCT) and the cove base. Abatement contractor shall be responsible properly disposing of all abated materials. Coordinate with a 3rd party California certified Industrial Hygienist to have air clearance test samples taken. Plastic containment shall remain in place until the test results written documents are presented to the Structural Supervisor showing that the air sample results test negative for ACM. Once air sample results come back clear, remove and dispose of plastic containment. Carpet and Cove Base installation Install approximately 660 square feet of new carpet squares, Achromatic, #5T1077, Color: Black, along with Tarkett cove base, Color: Pewter, in rooms 2A-52A, 2A-54, 2A-57, and 2A-59. Install all rubber reducers or transition strips, color Black, anywhere there is a change in elevation or flooring materials in the doorways of the Neurology Suite. Dispose of any trash, demolished or unused materials. Move all furnishing and non-attached equipment back to its proper place. PERIOD OF PERFORMANCE: All work must begin within (45) days of contract award. Work will be coordinated with all required parties, including, but not limited to, contractor, VA Loma Linda Heath Care Services (VALLHCS), Global Environmental Management Services (GEMS) Coordinator, Safety, Contracting Officer s Representative (COR), Chief, Maintenance and Operation, and Structural Supervisor. Work timelines may be subject to change due to VALLHCS operational requirements. Contractor must be able to perform work with no additional charges within (30) days of contact by the designated COR. PLACE OF PERFORMANCE: VA Loma Linda Medical Center 11201 Benton St Loma Linda, CA 92357 Neurology Suite, 2A-53A LICENSING REQUIREMENTS: Offeror must possess a 30-hour Occupational Safety and Health Administration (OSHA) safety card. Must be a vendor in our system and System for Award Management (SAM). SPECIFIC REQUIREMENTS: Contractor shall schedule start date with POC or designee five (5) business days in advance. Contactor personnel shall have valid Loma Linda PIV badges before starting work. Contractor shall coordinate energy source LOCK OUT /TAG OUT procedures with VALLHCS Mechanical Supervisor before starting work. When work is complete, Contractor shall notify the VALLHCS Mechanical Supervisor that energy sources will be restored and require verification. Contractor s supervisor or lead technician are to check in with VALLHCS Mechanical Supervisor, or in their absence, with Graphics Control prior to work each day Contractor shall conform to the following standards, policies, regulations, governing agency, and any other local, state, or Federal guidelines that are not listed here OSHA Regulations, www.osha.gov NFPA, www.nfpa.org RECOGNIZED HOLIDAYS: The following is a list of all Government recognized holidays: New Year s Day Labor Day Martin Luther King Jr. s Birthday Columbus Day President s Day Veteran s Day Memorial Day Thanksgiving Day Juneteenth Christmas Day Independence Day If a holiday falls on Sunday, the following Monday shall be observed as the legal holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a legal holiday by U.S. Government agencies. Also included, would be any other day specifically declared by the President of the United States of America to be a National Holiday. CONTRACTOR EMPLOYEES: Contractor personnel shall present a neat appearance and be easily recognized as Contractor employees. Examples: wearing of distinctive clothing such as uniform, badges, patches, etc. Contractor personnel shall wear identification (I.D.) badges during the entire time they are on the VA Loma Linda Healthcare System grounds. I.D. badges must have identification picture, name of the individual, and the represented company depicted on it. XI. SMOKING AND VAPING POLICIES: Smoking and vaping are prohibited on Government property X11. SAFETY AND PPE EQUIPMENT: Personal Protective Equipment shall be provided for each Contractor employee by the Contractor. Possession of weapons are prohibited. XIII. PARKING: It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Security Service. The VA shall not invalidate or make reimbursement for parking violations of the Contractor's personnel under any circumstances.
