- Description
- The Defense Health Agency Contracting Activity, Southeastern Markets Contracting Division intends to procure a firm fix price contract in support of US Naval Hospital Guantanamo Bay�s (USNHGB) Sterilizing Processing Department (SPD) and Dental Department. The contract will consist of a base Period of Performance (POP) of 12 months and four Option periods of 12 months each. The POP start date is February 01, 2024. Scope: The contractor shall perform on-site preventive maintenance, preventive maintenance required parts replacement and installation, calibration, corrective maintenance (repairs), and field service reports for STERIS washer disinfector and sterilizer equipment, AMSCO� Century� 26x37.5 Medium Steam Sterilizers, InnoWave Unity Ultrasonic Irrigator, Reliance Genfore Washer Disinfector, AMSCO 5052, Innowave Ultra+, and V-PRO 60 Low Temperature Sterilization System. When there are product recalls, the contractor shall process and install equivalent for each equipment and must coordinate with USNHGB�s Biomed Medical Repair Division point of contract before installation of recalled equipment or parts. The services provided per equipment shall be in accordance with the Original Equipment Manufacturer (OEM) specifications and the Joint Commission Hospital Accreditation Program requirements. The contractor shall provide OEM Certified Field Engineers and Technicians who have gone through OEM training to perform services on the STERIS equipment. The contractor shall provide training certificates prior to the performance start date of any service(s) under this requirement. Training certificates and/or certifications shall detail training location, trainee�s name, training completion date, and training expiration date. Place of Performance:� The services must be provided at the US Naval Hospital Guantanamo Bay, Cuba. To be considered for award, firms must be registered in the System for Award Management (SAM).� Offerors may obtain information on registration in SAM by calling 866-606-8220, or via the Internet at https://www.sam.gov. Responsible sources may identify their interest and capability that shall be considered by the agency. This notice is NOT a request for competitive proposals. A determination by the Government not to compete the proposed requirement based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Capability Statements: To determine market interest, capable businesses are invited to submit capability statements by the date indicated in this announcement. Capability statements shall be on Microsoft Word or Adobe Acrobat format, must not exceed five pages, and shall include the following items: (1) A company profile including CAGE Code and DUNS number and a statement regarding its representation status in SAM.gov. (2) A complete description of the offeror's capabilities, related experience, facilities, techniques or unique combinations of these factors that clearly relate to the Government's specific needs for� this requirement as identified above. �(3) A summary of at least two specific past performance on same or similar scope of work performed by the Offeror within the past three (3) years, including a description of the services provided, the dollar value of the effort, and the contract number. Capability statements shall be submitted via email to Yuleiny.garciacontreras.civ@health.mil and lenore.y.paseda.civ@health.mil, no later than the due date and time stated in this sources sought. Capability statements provided after this time may not be included for competition considerations. Email should reference - Source Sought STERIS Maintenance Services - in the subject line.
