- THIS IS THE ORIGINAL SOLICITATION REFERENCING FORMER DRAFT PRE-SOLICITATION�FA440724QH001(DRAFT).� **Please see attachments for details about the solicitation. The 375th Medical Group (375 MDG) at Scott AFB IL requires full Emergency Medical Services (EMS)/Advanced Life Support (ALS) response and patient transport services for Scott Air Force Base (SAFB), to include but not limited to, all work/office building locations, hangars, flight line, recreation areas, Airmen Dormitories and on-base family housing areas (Patriots Landing, Shiloh West, Shiloh East, Colonial & Georgian). Respond to all installations EMS requests and participate in base exercises, special events, specialized training and in-flight emergencies. Full range of EMS/ALS and ambulance services IAW installation protocols/standing orders and staffing minimums and IAW current and future state guidelines, regulations and/or protocols. The following factors will be used in evaluation process: Factor 1 - Technical Approach Factor 2 � Past Performance Factor 3 � Price Regarding the order of importance, the relative importance of each factor is as follows: Past performance is more important than Price and Technical.� The Government will select the best overall quote, based upon an integrated assessment of Technical (Factor 1), Past Performance (Factor 2), and Price (Factor 3).� However, to be eligible for award, the offeror shall have an acceptable rating in each Factor being rated acceptable/unacceptable. This requirement is deemed Mission Essential; therefore, the offeror is required to submit a Mission Essential Plan, which will be evaluated by the Government on an acceptable/unacceptable basis. Questions and Answers/Assumptions and Exceptions: Contractors are cautioned that all communications are to be channeled through the Contracting Officer (CO). Any questions, whether technical or contractual in nature, shall be submitted in writing. Questions and Answers shall be submitted in writing to the Contract Specialist (CS), Mr. La�marr Watson, lamarr.watson.2@us.af.mil, and the CO, Capt Sun Jo, sun.jo.1@us.af.mil. All questions shall be submitted by 12:00 PM Central Time, on 12 January 2024. Questions received after this time may not be answered. Quotes shall be submitted to the Contract Specialist (CS), Mr. La�Marr Watson, lamarr.watson.2@us.af.mil, and the Contract Officer (CO), Capt Sun Jo, sun.jo.1@us.af.mil no later than 4:00 PM Central Time on 2 February 2024. Late responses will not be accepted. **AMENDMENT A01 HAS BEEN ATTACHED. PLEASE SEE ATTACHMENT 08 BELOW. -The purpose for Amendment A01 is to provide responses to the questions asked regarding this SAQR.
