- STATEMENT OF WORK 1. Background. The Oncology Department at the Captain James A. Lovell Federal Health Care Center (FHCC) requires door to door Priority Overnight Express to send and receive imaging discs/and pathology slides from outside hospitals as industry standard to avoid delay of care. 2. Scope. Contractor shall provide all management, supervision, personnel and labor, materials, equipment and supplies necessary for the shipment for imaging discs and pathology slides to the FHCC located in North Chicago, Illinois, in accordance with the terms and conditions of this statement of work. The Contractor shall be responsible for all licenses or certifications necessary in order to perform these services. All work shall be performed in accordance with the following specifications. The Oncology department at the Captain James A. Federal Health Care shall require the contractor to provide door to door Priority Overnight Express Delivery of imaging discs and pathology slides to different destinations throughout the Continental United States. The Oncology department shall package all envelopes/boxes and will label them accordingly as to the contents of the package. The estimated number of packages shipped per week is between 10-20 boxes and/or 40-80 envelopes per month. Pick up times shall be Monday through Friday, any time after 14:00 hour (2:00pm) Central Time. Packages shall be shipped to the destination as noted on the shipping label on the package. Packages will be picked up at the following location: Federal Health Care Center 3001 Green Bay Road North Chicago, IL 60064 Bldg. 133CA - Basement Room BD127 Lab Specimen Window Phone: 224-610-2120 Contractor shall also provide the following customer support services: Ability to print shipping labels on demand via internet 24 hours per day Ability to track the shipment via internet Ability to produce shipping history reports Ability to receive notification via email when shipments are pick-up and delivered Ability to create a return label on-line with all shipments so that human blood boxes can be returned and reused Supply labeling materials Notify customer through email of any shipping issues 3. Performance Monitoring A staff member from the Oncology department will be designated as the Contracting Officer s Representative to represent the Contracting Officer in furnishing technical guidance and advice regarding the work being performed under the task order. The foregoing is not to be construed as authorization to interpret or furnish advice and information to the Contractor relative to the financial or legal aspects of the task order. Enforcement of these segments is vested in and is the responsibility of the Contracting Officer. 4. Security Requirements Contractor Delivery Personnel: All drivers assigned by the contractor to perform services shall be proficient in written and spoken English (38 USC 7402). Contractor shall be responsible for ensuring that all delivery personnel are trained and competent in the areas of transportation of blood and blood products. Training shall include compliance to regulations for transport of biohazards, use of rigid containers when appropriate, temperature control, notification procedures in case of accident or spills, proper procedures for handling and disposal of spillage, etc. Contractor shall make available documentation of training upon request. Contractor Delivery drivers shall be pre-screened by the contractor and must be able to obtain proper security credentials for access to the Federal Health Care Center. Contract delivery drivers shall always wear uniforms and identification badges while on FHCC property.
