- Combined Synopsis/Solicitation Notice Combined Synopsis/Solicitation Notice Page 3 of 73 Combined Synopsis/Solicitation Notice *= Required Field Combined Synopsis/Solicitation Notice Page 1 of 73 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR Part 13.5 for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2024-03 (effective 02/23/2024). This competitive solicitation is being issued as a Total (100%) Small Business Set-Aside. The associated North American Industrial Classification System (NAICS) code for this procurement is 811210 with a small business size standard of $34.0 M. Place of Performance: Michael E. DeBakey VA Medical Center, 2002 Holcombe BLVD. 77030 Anticipated Performance Period: May 1, 2024 to April 30, 2025 plus four 12 Month Option periods at the discretion of the government to exercise. All questions pertaining to this solicitation shall be submitted by 9:00 AM CST April 26, 2024. Send all questions electronically to arlene.blade@va.gov . NO TELEPHONE CALL WILL BE ACCEPTED. Only electronic offers will be accepted. Offers shall be submitted to arlene.blade@va.gov NO LATER THAN April 30, 2024 by 1:00 PM CST. Quoters shall include Solicitation Number 36C25624Q0763 in the subject line. Contractor is required to be actively registered in the System for Award Management (SAM) and Small Business Administration (SBA). SDVOSB concerns shall be actively registered and verified in the Small Business Administration VetCert website. Quotes may be considered non-compliant and rejected if the Contracting Officer is unable to verify active registration status. Contractors are required to provide supporting documentation that you can perform the required services and obtain OEM parts. Contractors must comply with the Limitation on subcontracting. Contractors must identify which required services in the Statement of Work they will actually perform in accomplishing this requirement. The Michael E. DeBakey VA Medical Center is seeking a contractor to provide all management, tools, supplies, equipment and labor necessary to provide comprehensive preventive maintenance services, comprehensive corrective maintenance services, Original Equipment Manufacturer (OEM) repair/replacement parts, and support services for government owned Histology Lab equipment in accordance with the OEM recommendations for Preventive Maintenance and service manual and the Statement of Work.
