- The purpose of this amendment is to extend the closing date and include scope of work (SOW) revisions pertaining to Solicitation 36C24224Q0851. Quote submissions shall now be submitted on or before Thursday, August 15th, 2024, 12:00 PM EST. B.2 SCOPE OF WORK B.2.1 GENERAL This firm fixed price requirement is for the annual cleaning of approximately 1,300 exterior windows located at the Syracuse VA Medical Center (800 Irving Avenue, Syracuse, NY 13210) from 10/01/2024 to 09/30/2025, to include four (4) option years. This service requirement shall include the cleaning of all ground through fourth floor windows. The contractor shall provide all necessary labor, materials, supervision, and travel to perform the services specified in the scope of work (SOW). Services vital to the continuation of this requirement shall be added only after authorization and written modification from the contracting officer. B.2.2 SCHEDULE All work shall take place during normal business hours unless otherwise specified by the VA: Monday-Friday, 7:00am to 5:00pm, excluding Federal Holidays (New Year s Day, Birthday of Martin Luther King, Jr., Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and, Christmas Day). This service shall occur at some point between the months of June and September. All work shall be coordinated with and accepted by the COR at least seven (7) days in advance. B.2.3 ANNUAL EXTERIOR WINDOW CLEANING REQUIREMENTS The contractor shall clean all exterior and storm windows thoroughly, leaving them translucent and free of hard water stains, calcium build-up, dirt, streaks, tar, gum, paint, water proofing compound and other substances. The contractor shall clean all outside sills on exterior windows to include cleaning between screen and window ensuring that they are free of dirt, tar, gum, waterproofing compound, and other substances. B.2.4 CONTRACTOR OWNED VEHICLES The contractor shall be responsible for maintenance of all company equipment and trucks. Contractor equipment shall be parked/staged in a designated area on the far-right side of the warehouse loading dock located in the rear of the building. An aerial man-lift which will be required to access the third floor of the facility, shall be provided by the contractor for each annual on-site service until all work is completed. Contractor vehicles shall not dispose of or drain off liquid (or any accumulation of chemicals that may be harmful to the environment) on VA Medical Center grounds. In the event of any spills of oil or other fluids caused by the malfunction of the engine or hydraulics, contractor personnel shall immediately clean-up in accordance with Environmental Protection Agency (EPA), Department of Transportation (DOT), and Occupational Safety and Health Administration (OSHA), NY State Department of Environmental Conservation (DEC) and any other state and federal regulations/guidelines. B.2.5 SAFETY The contractor shall provide and use all necessary tools, equipment, and/or Personal Protective Equipment (PPE) to perform work safely, effectively, and timely. The contractor shall use ladders or aerial man-lifts and shall not stand on frames or operator bars while servicing windows under this requirement. Aerial lifts shall comply with OSHA Standard 29 CFR 1926.453. The Contractor shall supply and use a properly rated counterweight ballast any time contractor personnel is tied-off performing work. No tools or equipment shall be left unattended. All work areas shall be clean and left in original condition at the close of each workday. Contractor shall take all precaution necessary to protect persons and property from injury or damage. Any chemicals anticipated for use in this service contract shall be annotated in the vendor proposal via Safety Data Sheet (SDS). The SDS shall notate how each chemical will be contained. Contractor shall ensure no damage is caused to VA property due to chemical usage. If any chemicals damage VA property, the contractor shall be liable for repairs at no additional cost. The contractor shall be responsible of insurance and compliance to all state and federal guidelines. For safety of contractor employees and VA property, only established roads/drives shall be used in performance of this contract. The Government shall not be held responsible for any injury caused to contractor employees. B.2.6 REPORTING The contractor shall report daily to the Environmental Management Service office, before and after each workday to complete a sign-in/sign-out log detailing the planned work/actual work. A complete digital report (PDF File) of all work performed shall be emailed to the COR within five (5) business days of completion. Information outlined in the report shall include (at minimum) the following: Company name, address, and telephone number. VA contract number, PO number, name of COR/CO. Date and time of work. Detailed quantity of each contract line-item used, not used, or expected to be used in upcoming visits. Reference materials (if applicable). Recommended preventative maintenance and/or corrective action (if applicable). B.2.7 SITE MAP 1.
