SECTION D - CONTRACT DOCUMENTS, EXHIBITS, OR ATTACHMENTS D.1� STATEMENT OF WORK Eastern Kansas VAMC Audiology Sound Booth Purpose: Eastern Kansas has a requirement for one (1) sound booth for testing in the audiology clinic in St. Joseph for activation. Full services will be available with the new CBOC building. A sound booth is the gold standard for audiology testing. Requirements:� One (1) Sound Booth- Custom Eckel Sound Room Requirements and evaluation factors are as follows STANDARD AUDIOMETRIC ROOM/BOOTH FEATURES & OPTIONS SEE DRAWINGS ON FOLLOWING PAGES FOR OPTIONS TAKEN 1. OPTIONAL 32""W BY 73 3/4""H CLEAR OPENING DOORS ""C/O"", 36""W BY 73 3/4""H C/O DOORS & 36""W BY 80""H C/O DOORS - SPECIAL SIZED DOORS UPON REQUEST. 2. OPTIONAL 20""W BY 22""H OR 20""W BY 60""H DOUBLE GLAZED ACOUSTIC WINDOWS AVAILABLE FOR DOORS - SPECIAL SIZES UPON REQUEST. 3. STANDARD 20""W BY 30""H OR 24""W BY 30""H DOUBLE GLAZED ACOUSTIC PATIENT VIEWING WINDOW - SPECIAL SIZES UPON REQUEST. 4. STANDARD CARPET OR CARPET TILE SUPPLIED. 12. EMPTY UTILITY BOXES WITH PULL WIRES CAN BE PROVIDED FOR UTILITIES SUCH AS DATA, ALARMS & THERMOSTATS UPON REQUEST. 5. Specific Tasks.� 5a. Engineering Focus Objectives: Design, develop, integrate, test, deliver, install and provide warranty support for one (1) sound booth in St. Joseph, MO. Meet or exceed all specifications, testing, and criteria found in the Department of Veterans Affairs Booth Audiometric Examination Specifications document (attached).� Must also meet or exceed ANSI S3.1-1999� (or later standards):� Maximum Permissible Ambient Noise Levels for Audiometric Test Rooms. Audiology sound booth- One (1) single walled sound booth. All electrical components shall be UL (or equivalent laboratory approved) listed Hospital Grade components and shall operate on 110 V, 60 Hz, single phase power.� Each booth needs numerous electrical outlets to support standard equipment to be installed. Each audiometric booth must have a dedicated 20 amp circuit for power. If the lighting is on a separate circuit, then the lighting and power need to go to a single source ground. Before booths are delivered, the contractor will provide the power requirements to Eastern Kansas VA so that the Engineering team can assess the functionality and viability. Carpeting used must be static resistant and durable, hypoallergenic materials preferred. Carpeted or soft material used on side panels within suites is desirable, as they would increase attractiveness of environment for plane tree objectives, but not absolutely necessary as they do not effect test results.� Suites must have adequate ventilation for comfort and connect to building HVAC.� Suite shall be provided with a ventilation system designed to be connected to the facility building HVAC systems, and provide vibration isolation from the duct work of the building HVAC system, to prevent vibration/noise affecting booth sound quality. Awarded contractor must provide Project Engineering with a set of Shop Drawings within 30 days of award which show all connections to building systems. Deliverables:� Successful delivery of Audiology Booths to the St. Joseph CBOC at the new CBOC location. � 6. Performance Monitoring Not applicable.� This is a contract to purchase audiology suites and booths. 7. Security Requirements There is no security requirements for this contract.� 8.� Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). � There is no government-furnished equipment for this contract.� 9.� Other Pertinent Information or Special Considerations. ����� a.� Identification of Possible Follow-on Work.� Not applicable ����� b.� Identification of Potential Conflicts of Interest (COI).�� Not applicable ����� c.� Identification of Non-Disclosure Requirements. Not applicable ����� d.� Packaging, Packing and Shipping Instructions.� Not applicable ����� e.� Inspection and Acceptance Criteria.�� Safe and timely delivery of audiology suites and booths to the basement of Building 500 in the Audiology Clinics.� Equipment should be fully functional, unbroken, etc. 10. Risk Control 11.� Place of Performance. The booth will be delivered to the� St. Joseph CBOC. 12.� Period of Performance.� No later than December 2024. Delivery Date: November 2024.
