Purpose of the project Support the new seismically inadequate SCI building Background VA San Diego Healthcare System provides medical and primary care services to qualified Veterans at a main hospital. VA San Diego Spinal Cord Injury and Community Living Center has executed a new build that will meet the VA seismic criteria and meet VHA standard for long term care. This facility, located at 3350 La Jolla Village Drive, San Diego, CA 92161, has a requirement for outdoor fitness equipment for SCI patients. The outdoor fitness equipment will enhance rehabilitation services for the SCICLC patients. It will include various pieces of outdoor equipment, as listed below. Scope and methodology The expansion of capacity and Services provided was planned to meet users need. This will be new outdoor equipment for physical therapy. Introduction - Veteran Affairs San Diego Healthcare System has a requirement to purchase outdoor physical therapy equipment to extend the inpatient mobility rehabilitation program. This requirement is in support of the activation of VASDHS new SCI/CLC building and the Jennifer Moreno medical center. In addition, the vendor will provide service manuals that includes theory of operation, maintenance requirements, repair information, schematics, part lists and troubleshooting information in electronic format. Date of manufacture must not be greater than 1 years before the purchase order date. The equipment must not be used or refurbished and must have the latest manufacturer s software/hardware updates installed. The following Addresses are applicable Delivery SCI building, VA Medical Center 3350 La Jolla Village Drive, San Diego Ca 92161 The required delivery window is 60 days ARO. The awardees actual delivery dates will be confirmed by VA upon award. Post-award, the vendor will coordinate delivery identified in this document with the VA San Diego POC. Once the dates have been confirmed, modifications are subject to written approval by the Contracting Officer. Schedule changes that are beyond the dates specified in the contract will require approval by the Contracting Officer via a contract modification. Delivery and receipt of the proposed items is anticipated to be directed to the location identified in 2.b.i. above. Confirmation of delivery location will be provided post-award. To coordinate delivery, contact VASD POC Xiaochen Li at (858)642-6324 Xiaochen.li@va.gov The delivery of items identified in this document shall be pre-coordinated 72 hours in advance with the POCs and take place during normal business hours which are defined as: 8:00 AM to 3:00 PM Pacific Daylight Time, Monday through Friday, and excluding Federal Holidays. Labeling of delivered items shall include the VA facilities contract number and VA purchase order number for identification and reference upon receipt of product. The vendor shall protect all finished spaces and surfaces as required from delivery damage. The vendor shall use covering and protection to the extent necessary to prevent damage to finished spaces. Any damage occurred during delivery is the responsibility of the vendor. The vendor will be responsible for paying for and repairing any damage or noted deficiencies to finished spaces and surfaces that occur as a result of the vendor s (or associated sub-contractors) delivery. All instructions provided from VA San Diego HCS POCs in 2.e. above to the representative will be binding as if given to the vendor s main contact. The VA San Diego HCS may provide specific instructions, however, only the Contracting Officer may change the terms or conditions of the contract. Delivery shall be 30 Days after receipt of order (ARO).
