- THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR QUOTE. The Network Contracting Office (NCO) 12, Great Lakes Acquisition Center, 115 S. 84th Street, Suite 101, Milwaukee, WI 53214, is performing market research to determine if there is a sufficient number of qualified: (1) Service Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); or (3) Small or emerging small business firms, or (4) Other Than Small business firms, to provide Optometry Chairs at the Illiana VA Medical Center located in Danville, IL. Brand Name or Equal: Haag-Streit Reliance 6200 Manual Pneumatic Recline Exam Chair and Reliance 942 Chair Mover Salient Characteristics: Exam chair: -Fingertip switches, as well as corded foot switch, that automatically control chair elevation, recline, and auto-return functions. -Hydraulic system for smooth, quiet reliable adjustments -Single-handed headrest -Foot activated rotation lock -Must be compatible with wheelchair glide system -Not to exceed 46 length fully reclined, 24 in width (arm to arm), and 24 width of base Exam Chair Glide System: -Must easily move a heavy exam chair for comfortable wheel chair access -Wide surface-plate with form-fitting rubber pad that provides stable positioning of any mounted exam chair -Foot-activated release mechanism -Sturdy, solid construction for long-term reliability -Must be compatible with exam chair purchased All interested firms who can meet the requirements stated above should respond, in writing, INCLUDING ALL THE FOLLOWING: Company name.
