Description
This is a Sole Source action for an acquisition for the Defense Health Agency (DHA) TRICARE Program Management Office pursuant to FAR 6.302-1 � �Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements.� TRICARE Program Management Office is requesting approval of this Justification and Approval (J&A) to add in-scope increase to award HT001124C0009 for Accelerating Care Transformation support services to limit consideration for award on sole source basis. HT001124C0009 was awarded on 29 March 2024 on a sole source basis under JA 24-0079. This action will be limited to the awardee for HT001124C0009, TransformCare, 25714 Ridge Road, Damascus, MD 20872. The Government intends to negotiate a firm-fixed price contract modification for this scope increase with TransformCare Inc. Contract HT001124C0009 is a directive from the Senior Director Level for an Accelerator Initiative that delivers high-value, patient-centric care that seamlessly integrates military and private sector care in support of readiness and health outcomes. This contract provides DHA with the flexibility to adjust health benefits, adapt care delivery models in support of evolving mission requirements and changes in American health care. Networks complement the capabilities of the Military Treatment Facilities (MTF) where they exist, and contractors ensure MTFs have the appropriate clinical capabilities and quality outcomes are offered as the first choice for eligible beneficiaries. The Military Health System (MHS) has recently undergone significant changes, as mandated by the Fiscal Year 2017 National Defense Authorization Act (NDAA). These changes involve an organizational restructuring, with (DHA assuming administrative responsibilities for all MTFs. MHS is poised to launch an ambitious digital health strategy, in service of its vision to care for joint forces. This includes budgeting, information technology, health administration, policy and procedure, military medical construction, and other relevant areas. Current support includes establishing start-up site objectives and scope, developing detailed implementation plans, location and population selection, project monitoring and control, data collection, reporting, and post-implementation analysis. Defense Health Agency (DHA) seeks to achieve the following objectives: Strategic Advisory Support -� Provide subject matter expertise in healthcare delivery, value-based care, clinical implementation, and health information technology (IT) to advise and support the Executive Director and DHA leadership through the Digital First transformation and related strategic initiatives. Pre-Care and Care Interface Design - Develop the functional requirements for Digital First pre-care and care interfaces to enhance patient access while prioritizing digital engagement and interactions. Identify potential digital interface points with existing traditional modes of care to leverage existing technology and workflows. Potential focus areas include telehealth, virtual care, eConsults, open access preventive care and others. Primary Care Model Design and Execution at Four Concurrent, Discrete Sites � Diagnose & Analyze Current State: Through diagnostic activities, document the current state of the primary care model at pilot sites and document opportunities for implementation of Digital First initiatives. Design and Implement Four Discrete, Concurrent Pilots: In design, will establish pilot objectives, define scope, develop detailed implementation plans, and facilitate implementation in collaboration with key stakeholders. Behavioral Health Solution Implementation - Document the current state and design implementation of a technical solution to improve on behavioral health outcomes for beneficiaries at pilot sites to be determined. For the additional scope, the contractor will provide research, planning, and implementation to expand care transformation efforts into specialty service lines, further enabling a patient-centered, digitally-enabled care delivery system. This includes the following: Strategy modeling for the digital health and accelerator initiatives to ensure success and adequate return on investments made in support of DHA strategy. Define a performance evaluation plan for the venture sites, including clear and relevant performance metrics tied to value creation. Expand adoption of virtual modalities of patient care into specialty service lines where appropriate and beneficial for the patient to increase patient convenience. Establish more effective and efficient coordination of care (e-Consult) between primary care and specialty service lines, as technology allows, to reduce unnecessary referrals, leakage to the network and increase patient convenience by meeting their needs efficiently. Per HT001124C0009 Performance Work Statement (PWS) 1.5.1 and 1.5.3: �1.5.1: Strategic Advisory Support � The contract team will provide subject matter expertise in healthcare delivery, value-based care, clinical implementation, and health information technology (IT) to advise and support the Executive Director and DHA leadership through the Digital First transformation and related strategic initiatives. 1.5.3: Behavioral Health Solution Phase 1 Implementation at Locations � The contractor will support the implementation of a technology solution to generate improved behavioral health outcomes at four disbursed geographical locations. Support will include analysis, process design, training and change management support, data collection, reporting, and post-implementation analysis. Behavioral health solutions will be executed in parallel as primary care model design and testing in task 3.� The requested additional scope is only for the existing venture sites. No new venture sites will be added to the current contract. Based on the size of the current MTF, DHA is requesting additional support for the same scope as noted in HT001124C0009 PWS 1.5.1 and 1.5.3. The scope increase is a continuation of subject matter expertise in healthcare delivery through adjusting the support to the appropriate levels based on the size of the current venture site to meet Lieutenant General Crosland�s goal of the Digital First transformation.
