Amendment to a Previous Solicitation Page 1 of Amendment to a Previous Combined Solicitation Page 4 of 4 Amendment to a Previous Solicitation *= Required Field Amendment to a Previous Solicitation Page 1 of 4 Final Questions and Answers Question 1: When does the solicitation close? Government Response: All questions were due before 10/17. All proposals are due Monday 10/21 at 3:00pm MST. Question 2: We would like to know if there is (was) an incumbent performing these services. Government Response: Stericycle, Inc. Question 3: Could you please provide the current / previous contract number? Government Response: 36C25919C0051. Question 4: SOW indicates Attachment A to show where collection and accumulation points are located in RMRVAMC. Please provide attachment A and/or advise. Government Response: It should read Appendix A. Attachment A is included at the end of the solicitation document, rather than attached. I have relabeled it as Appendix A in the solicitation document. Question 5: What is the cost for the PIV card? Government Response: There is no PIV cost if vendors obtain the PIV card from a VA location. Please confer with the COR (Contract Officer Representative) on site for details. The intent of the language in the SOW is to direct vendors to the COR for PIV assistance. Question 6: Could you please provide a list of the specific DEA Schedule Controlled substances we will be handling for this project? Government Response: The disposal system should be possible of neutralizing most DEA controlled substances. Insofar as this is a hospital contract, vendors should be prepared for drugs on the standard DEA schedule. Manufacturer s literature should be readily available for that. The VA hospital will not make a public publication listing all their DEA hazardous drugs. Note: No additional questions will be answered at this time.
